HIFU Treatment for the Neck —
Firmer, Within Limits

HIFU treatment for the neck is aimed at one of four common neck changes: skin and platysma that have loosened under the jaw and at the sides. Necklace lines, crepey texture and vertical bands each need different care, so this page sets out where focused ultrasound fits in a neck plan.

The Mechanism

Thin Skin Over A Thin Muscle

Book a Consultation

Neck skin is thinner than facial skin, with fewer oil glands and less fat, and lies directly over the platysma. Four changes tend to arrive together: necklace lines, crepey texture, laxity under the jaw, and vertical bands.

Focused ultrasound heats small points in loosened skin and platysma, which remodel over 8–12 weeks. It reaches only laxity. Necklace lines are creases from bending and screen posture that heat cannot unfold; crepey skin can look thinner and shinier after heat alone; bands are muscle.

At A Glance

Why The Neck Gets Lighter Settings

Dr Sin Yong, Global Key Opinion Leader in Facelifts & Body Contouring, uses the 3.0 mm depth for most of the neck and 1.5 mm over the thinnest skin at the front. The 4.5 mm depth is kept for under the chin and just below the jaw, where a pinch shows enough tissue for it to land in the platysma. Energy is stepped down from facial settings, and nothing is placed over the thyroid or front midline.

Who It Suits

Which Neck Suits HIFU Treatment

The neck that suits HIFU has early to moderate laxity: softened skin under the jaw, a blunted jaw-neck angle, and a thin fold of skin rather than fat, without heavy bands or hanging skin. It is often treated with the lower face, so the jawline does not end up firmer than the neck.

The Decision

Pairing Laxity Work With Skin Work

Most neck plans pair a laxity step with a skin-quality step.

  • Lines and crepe: skin boosters or fine filler, through neck and hand revitalisation, plus daily sunscreen.
  • Vertical bands: a conservative dose of BTX along each band; see neck bands.
  • Laxity with compact fat under the chin: the Time Freeze Laser LCLR is considered.
  • Laxity in thinner tissue: radiofrequency, such as Thermage FLX.
  • Much loose skin, or a very lean neck: HIFU is unlikely to help; surgical referral.

On The Day

A Neck HIFU Appointment

  1. Photographs with your consent, head level and turned, for your medical record only.
  2. Questions about thyroid disease, neck surgery, implants, filler or threads.
  3. Numbing cream, removed before treatment.
  4. Each zone pinched to set its depth; the thyroid and front midline left unmarked.
  5. Lines across the front and sides and up under the jaw, with energy adjusted to what you feel.

The transducer is repositioned often, as contact is harder on a curved neck. See what to expect at a HIFU visit.

Afterwards

Recovery And Aftercare

Tenderness eases over days; see HIFU downtime and HIFU aftercare. Keep sunscreen on the neck too. The result is judged at about three months.

Safety

Risks On The Neck

  • Redness, tenderness and sometimes a bruise, which settle.
  • Too deep for the zone: heat in fat or deeper tissue, leaving a hollow or tender nodule.
  • Too shallow, or poor contact on the curve: a welt or burn.
  • Heat near the jaw edge can irritate the nerve to the mouth corner and weaken it for a time.

Contact the clinic the same day if a raised welt stays, a blister forms, a hard or painful lump appears, swelling is marked or the mouth moves differently; see HIFU complications.

FAQ

HIFU for the Neck — Common Questions

Ask a Question
No. Necklace lines are creases set into thin skin by bending the neck, and heat does not unfold a crease. They respond to skin-quality work, such as skin boosters or a very small amount of soft filler, with sun protection and less time looking down at a screen. HIFU can sit alongside if the neck is also lax.
No, and it can make them more obvious. The cords are edges of the platysma muscle, which soften when the muscle is relaxed with a small dose of BTX, not with heat. Firming skin over a prominent band can outline it, so bands are treated separately.
Most of the neck lacks the depth for it. The 4.5 mm focal point needs enough tissue to land in the platysma, not in fat or deeper structures. That is often true just under the chin and jaw, rarely across the front and sides.
Often, so the jawline and neck change together. The neck still gets its own map, depths and lower energy, planned from the start rather than added at the end with whatever lines remain. Whether both areas suit HIFU is decided at the examination.
Not necessarily, but tell Dr SY about it, and about any thyroid condition, at consultation. HIFU lines are never placed over the thyroid or down the front midline; treatment stays at the sides and under the jaw. Whether your history changes the plan, or means waiting, is decided after examination.
