Hands age faster than the face and are almost never treated. Restoring dorsal volume smooths the contour and softens the prominence of veins and tendons — a short treatment with an immediate, disproportionate effect.
The back of the hand carries only a thin layer of subcutaneous fat. In youth it provides a smooth, cushioned surface over the extensor tendons and metacarpals. From roughly the fourth decade that layer depletes, and the underlying architecture becomes visible — tendons standing proud, metacarpal outlines defined, and the dorsal venous network increasingly prominent.
Filler placed in the correct plane restores that cushion. It does not remove veins or tendons — it pads the tissue around them, which reduces how sharply they read. The effect is immediate, and because the hand is not an expressive area, results tend to sit undisturbed.
Pigmentation is a separate matter. Solar lentigines accumulate on the dorsal hand faster than almost anywhere else, and no amount of volume restoration addresses them. Where both are present, Dr. Sin Yong sequences laser and volume rather than treating one and calling it complete.
