Insulated Needles · Dermal Coagulation · CityLink Mall
RF microneedling is not microneedling with a warming sensation added. Insulated needles carry radiofrequency energy past the epidermis and release it at a set dermal depth — which is the whole reason it can be used confidently in skin that pigments after injury.
A plain microneedling pen makes channels and nothing else. The injury is mechanical, it is shallow, and the healing response it provokes is proportionate to a small puncture. RF microneedling uses the needle as a delivery route rather than as the injury itself — the needle arrives at a chosen depth in the dermis and then radiofrequency current passes between adjacent needle tips, heating the tissue around them until protein denatures. What the skin repairs afterwards is a thermal lesion, not a puncture, and the healing response is correspondingly larger.
The insulation along the needle shaft is what makes this usable in Asian skin. Because current is only emitted from the exposed tip, the epidermis and superficial dermis that the needle passed through receive no direct energy. Heat stays in a discrete column at depth. Ablative fractional resurfacing works the other way round — it removes tissue from the surface downward, and every treated column breaches the epidermis. That breach is precisely what recruits melanocytes into an inflammatory response, and it is why ablative CO2 resurfacing in Fitzpatrick IV skin carries a real risk of post-inflammatory hyperpigmentation that can take months to resolve.
The dermal columns themselves behave the way any controlled wound does. Denatured collagen contracts immediately, which accounts for the slight tightening patients notice within the first week and which then largely relaxes. The durable change comes from what replaces it: fibroblasts migrate into the coagulated zones, lay down new collagen and elastin, and remodel that deposit over the following two to three months. Between the columns the dermis is untouched and acts as a reservoir of healthy tissue that drives repair — the same fractional principle that governs laser resurfacing, applied without an epidermal wound.
Depth is selected, not fixed. Superficial passes at around half a millimetre address textural irregularity and the collagen scaffolding immediately beneath a pore opening. Deeper passes of two millimetres or more sit at the level where acne scar tethering and dermal thinning live. Most treatments use more than one depth in a single session, stacked, because the problems being treated rarely occupy a single plane. On a face this is a device setting; on a decision it is the difference between a treatment that works and one that was aimed at the wrong layer.
Indications
RF microneedling has a specific working range and it is worth being precise about it. It is a dermal remodelling tool. It changes the quality and thickness of the dermis within the area treated, and it does that well. It does not move tissue that has descended, and it does not replace volume that has been lost.
The epidermis is not where the problem is. Insulated needles let you leave it alone and still work at depth.
Sequence
Assessment first, and it is not a formality. Active inflammatory acne is a contraindication to treating the same area — heating an inflamed dermis worsens the inflammation and, in pigmenting skin, guarantees marks. Isotretinoin within the recent past, a history of keloid formation, active herpes simplex in the treatment field, and pacemakers or implanted electronic devices all change the plan or rule it out. These are asked about before a device is switched on, not after.
A session takes forty to sixty minutes including topical anaesthesia, which is applied for around half an hour beforehand. The treatment itself is uncomfortable rather than painful and is well tolerated on the face. Afterwards the skin is red and feels tight and sunburnt for one to three days, with a fine grid pattern sometimes visible for the first day. Pinpoint bleeding at the time of treatment is normal and stops quickly. There is no peeling in the way an ablative laser peels, and most patients are presentable within forty-eight hours.
Three to four sessions at four to six week intervals is the usual arrangement for textural and scar work, with the outcome judged at three months after the last one rather than immediately after it. Sunscreen is not optional afterwards, particularly in Singapore, because a freshly remodelled dermis under unprotected UV exposure is how you convert a good result into a pigmentation problem. Where the presentation is predominantly scarring, RF microneedling is usually one component of a staged plan rather than the whole of it.
