Compartment Anatomy · Position · CityLink Mall
The jowl fat pad is a named anatomical compartment present in every face from childhood. It does not grow. It relocates — and that distinction decides whether the correct treatment repositions it or, far more rarely, removes it.
Facial fat is not a continuous blanket. It is divided into discrete compartments, each bounded by fibrous septa and each with its own blood supply, sitting in two broad layers — superficial fat above the SMAS and deep fat below it. The jowl fat pad is the most inferior of the superficial compartments, occupying the region just in front of the mandibular retaining ligament along the lower border of the jaw.
Because the compartments are separate, they age separately. Some deflate early and markedly, some deflate late, and some descend while their neighbours stay put. The visible landmarks of an ageing face — the nasolabial shadow, the jowl notch, the marionette line — form at the boundaries between compartments that are behaving differently. They are seams, not wrinkles.
The jowl pad has a specific relationship to the mandibular ligament, which anchors immediately behind it. When the SMAS and its retaining ligaments lose tension, the pad slides downward and forward and is stopped by that anchor. It bunches. The bulge a patient points to is the same compartment they have always had, now sitting lower and pressed against a boundary that will not let it continue.
This matters because the compartment is usually losing volume at the same time it is descending. A jowl that looks fuller is not a compartment that grew — it is a compartment that moved into a position where it casts a shadow and breaks a contour line. Measured against a photograph from twenty years earlier, there is very often less fat in the lower face, not more.
What Changes
Before any decision about reduction, the pad is characterised. The three presentations below look broadly similar in a mirror and call for entirely different management, and confusing them is the most common route to an aged-looking lower face after treatment.
The jowl fat pad is not a mistake the face made. It is a normal structure that stopped being held where it belonged.
The Decision
Injection lipolysis and surgical excision both reduce the pad, and both are occasionally correct. The question is whether the finding is excess or displacement, and that is settled by lying the patient down. Tissue that resumes a normal position when gravity is removed is displaced tissue, and removing a portion of it would simply leave a smaller pad sitting in the same wrong place.
There is also a timing problem that does not show up for years. The lower face continues to lose volume through the fifth and sixth decades. A pad reduced in a face that will keep deflating leaves a hollow that is harder to correct than the jowl was, because volume can be replaced but a natural contour cannot easily be rebuilt. This is the same logic that applies to buccal fat removal, and the regret pattern is the same.
Where the jowl is the presenting complaint but the mechanism is descent, the honest plan addresses the SMAS and the ligaments and leaves the pad alone. Where the jawline itself has lost bony definition, adding projection through chin enhancement frequently improves the appearance of the jowl more than anything done to the jowl. The pad is rarely the right place to intervene.
