HIFU Treatment for Jowls —
Firming, Not Repositioning

HIFU treatment for jowls heats the SMAS and skin along the lower cheek and jawline so the layers that let the jowl slide become firmer. It suits early to moderate jowling in a face with good volume. It cannot lift a heavy jowl that moves freely, take away the fat inside it or refill a flat midface.

The Mechanism

Why HIFU Firms Rather Than Lifts

Book a Consultation

A jowl forms where tissue from the lower cheek slides down and stops against the mandibular ligament, the anchor just behind the dip in front of the jowl. Loose SMAS and ligaments, thinner skin and lost midface volume feed it; jowls and lower face sagging explains the anatomy.

Focused ultrasound heats small points in the SMAS and skin over the jowl, which remodel over 8–12 weeks. The layers firm up, but HIFU does not carry tissue back up, remove fat inside the jowl or restore volume above it. In an early jowl that can be enough to sharpen the jawline; in a heavy one it is not.

Depths Along The Jawline

Dr Sin Yong, Global Key Opinion Leader in Facelifts & Body Contouring, palpates each zone first. The 4.5 mm depth goes into the SMAS of the lower cheek and jawline where tissue allows, then a 3.0 mm pass; thinner skin at the front of the jaw gets 1.5 mm. Lines follow the direction the tissue slid, avoid the nerve near the jaw edge, and keep 4.5 mm off thin tissue on bone.

Who It Suits

The Jowl That Responds To HIFU Treatment

HIFU suits early to moderate jowling with good volume and reasonable skin: the cheeks are not hollow, and the jowl does not swing when you turn your head.

The Decision

Order Matters For Jowls

Jowls are rarely one-layer problems, so HIFU is often one step of several.

  1. If deflation above is driving the descent, cheek support comes first, with cheek fillers or collagen biostimulators. Filler is not placed into the jowl itself.
  2. Fat inside the jowl is assessed for the Time Freeze Laser LCLR.
  3. Lax layers are firmed with HIFU or the VF Lift – Vertical Facelift, a radiofrequency protocol suited to laxity in the skin and fat beneath it.
  4. Where tissue needs repositioning, a thread lift can be planned in sequence with HIFU.

A heavy lower face with much loose skin is a surgical question, and Dr SY will say so.

On The Day

A Jowl HIFU Appointment

  1. Photographs with your consent, upright and from both sides, for your medical record only.
  2. The jowl checked with your head turned.
  3. Numbing cream, removed before treatment.
  4. Lines marked along the jaw and lower cheek, leaving the nerve's path clear.
  5. Deeper lines first, then the 3.0 mm pass, with energy adjusted line by line.

Afterwards

Recovery And Aftercare

Jawbone tenderness, slight swelling and sometimes a small bruise settle over days; see HIFU downtime and HIFU aftercare. Early firmness is swelling; the result is judged at about three months.

Safety

Risks Along The Jaw

  • Irritation of the marginal mandibular nerve can weaken the mouth corner or lower lip for a time.
  • In a lean face, heat over thin tissue can reduce fat and leave the lower cheek hollow.
  • Jawbone tenderness, a small bruise and slight swelling are common and short-lived.

Careful placement lowers these risks without removing them. Contact the clinic the same day about a welt that does not flatten, marked or one-sided swelling, numbness that persists, or any change in mouth movement; see HIFU complications.

FAQ

HIFU for Jowls — Common Questions

Ask a Question
No. HIFU firms the layers that let the jowl slide, which can soften an early jowl and clean up the jawline. It does not move tissue back, remove fat or remove loose skin. A fully descended jowl, or one that swings when you turn, needs threads or a surgical opinion.
The jawbone lies close to the surface, and a focal point near bone registers as a sharp, hot ache rather than warmth. Dr SY feels the tissue depth before choosing a setting, keeps the deepest setting off thin tissue on bone, and adjusts energy line by line to what you report.
If the midface has lost volume, usually yes. A deflated cheek lets tissue drift towards the jaw, so firming the jowl alone treats the end of the problem, not its start. If your cheeks are full and the jowl is early, the jawline can be treated directly. Dr SY sets the order after examining both.
The marginal mandibular nerve, a small branch that runs near the lower edge of the jaw and moves the corner of the mouth and the lower lip. Heat close to it can weaken them for a time, leaving the smile uneven. Lines along the jaw are placed with its course in mind, and any change in how your mouth moves should be reported the same day.
Not by going into the jowl, because adding volume to descended tissue makes the lower face heavier. Filler can help around it: deep in the cheek for support, or in the small hollow in front of the jowl so the jawline reads as one line. Filler changes the outline; HIFU firms the layers.
