Periorbital Assessment · Patient Selection · CityLink Mall
Most of what patients call a hooded eyelid is not a problem of the eyelid. Energy-based tightening helps a narrow and identifiable group very well, and does almost nothing for the rest — so the assessment that decides which group you are in is worth more than the treatment itself.
Nothing is removed in a non-surgical eyelid lift, and understanding that sentence settles most of the disappointment associated with the treatment. Focused ultrasound or radiofrequency energy is delivered into the fibrous septae and the superficial fascial layer of the upper periorbital region and lateral brow. At a sufficient temperature, collagen triple helices denature and shorten. That contraction is immediate and small. What follows over the next eight to twelve weeks is the more useful part — fibroblasts populate the treated zone and deposit new collagen, which thickens and firms the tissue that was heated.
The lateral brow matters more than the lid in most cases. The brow sits on a fat pad which is itself supported by fascia, and when that support weakens the brow tail descends. Because the upper lid skin is anchored above, a descending brow tail pushes lid skin downward and produces the hood. Patients describe this as heavy eyelids. Tightening the fascia that supports the lateral brow raises the brow tail by a millimetre or two, and a millimetre or two at the brow removes noticeably more hooding than the same millimetre applied to the lid skin itself.
The upper lid skin responds separately and more modestly. Where the dermis has thinned and lost elasticity but there is no genuine surplus of skin, gentle surface-level tightening improves the crepey quality and the slight overhang. Where there is a true surplus — skin that folds over the lash line and can be lifted and held between fingers — there is nothing for heat to do. Collagen contraction shortens tissue by a fraction of its length. It does not delete a centimetre of excess skin, and no amount of repeated treatment will make it do so.
Treating this region safely is a matter of where the energy stops. The globe lies immediately beneath, the levator mechanism runs through the lid, and the supraorbital and supratrochlear nerves emerge at the orbital rim. Energy is therefore kept to the bony orbital rim and lateral brow, with protective measures for the eye, and depths are chosen conservatively — the same restraint that governs focused ultrasound lifting anywhere near the orbit. A practitioner willing to treat inside the orbital rim in pursuit of a better photograph is taking a risk that belongs to the patient, not to them.
Selection
The examination that decides this is short and specific. The brow is supported manually to see how much of the hooding it accounts for. The lid skin is pinched to establish whether there is genuine surplus. The lid margin position relative to the pupil is measured. Those three findings, not a preference for avoiding surgery, determine what is offered.
A hooded eyelid is usually a brow that has come down. Treating the lid skin of a patient whose problem is brow position is energy delivered to the wrong structure.
Sequence
Assessment comes first and includes questions that have nothing to do with appearance. Sudden or asymmetric lid droop, droop that worsens through the day, double vision, or any change in visual field is a neurological or ophthalmological matter until proven otherwise, and is investigated rather than treated cosmetically. Thyroid eye disease changes the periorbital appearance and must be recognised. A history of laser eye surgery, dry eye or previous eyelid surgery also changes what is appropriate.
The treatment itself takes twenty to thirty minutes for the periorbital region. Corneal protection is used where energy is delivered near the orbital rim. Sensation ranges from warmth to brief deep discomfort at each pulse, and it is well tolerated without anaesthesia in most patients. There is usually no visible downtime — mild redness or a little swelling for a day, occasionally a small area of tenderness over the rim that settles within a week.
The early tightening seen in the first fortnight is not the outcome. It relaxes, and the durable change appears between eight and twelve weeks as new collagen is laid down. Results are assessed at that point, and a second session is considered then rather than assumed in advance. Where the brow tail has descended because the temple has hollowed beneath it, restoring that scaffold usually contributes more than further energy. Where the lid margin itself is low, no further sessions are offered.
