Cellulite Singapore —
Tethering, Not Fat

Cellulite is not caused by toxins and is not simply a surplus of fat. It is a structural problem — fibrous septae anchoring the skin down to deeper fascia while fat between them pushes outward — which is why losing weight so often leaves the dimples exactly where they were.

The Mechanism

Anchors, Not Excess

Book a Consultation

Beneath the skin, collagen bands called septae run from the underside of the dermis down to the deeper fascia, dividing the subcutaneous fat into compartments. They are structural — they hold the skin to what lies beneath it and stop it sliding around. In a person with cellulite, some of those bands have become short, thickened and inelastic, and each one pulls the overlying skin downward at a single point.

That is the whole mechanism, and everything visible follows from it. Where a septum tethers, the surface is drawn in and a depression appears. Between the tethered points, the fat lobules have nowhere to go but upward, so they push into the overlying dermis and produce the raised areas that sit between the dimples. The result is not a lumpy fat layer; it is a flat fat layer held down unevenly. Pinching the skin or contracting the muscle beneath exaggerates it because both increase the tension across those bands.

The sex difference is anatomical rather than hormonal folklore. In women the septae run predominantly perpendicular to the skin surface, so each band pulls straight down and the fat between them herniates straight up. In men they are arranged as a criss-cross lattice, which distributes tension and prevents the herniation pattern from forming. This is why cellulite is overwhelmingly a female finding, and why it appears in women who are lean, fit and young as readily as in women who are not.

Ageing makes it more visible without being its cause. The dermis thins, collagen density falls, and the skin loses the tensile quality that previously concealed what was happening beneath it. Nothing has changed in the septae at that point — the covering has simply become less able to hide the contour. This is also why any credible approach addresses both layers: the tether that creates the depression, and the dermal quality that determines how sharply it reads.

Structure One
Fibrous Septae
Collagen bands running from dermis to fascia. Where they are short and inelastic they anchor the skin downward at a point. This is the origin of the depression, and it is the only element whose release changes the depression itself.
Structure Two
Fat Lobules
The compartments between septae. Constrained laterally, they herniate upward into the dermis and create the raised areas. Reducing their volume softens the contrast but does not release what is holding the skin down.
Structure Three
Dermal Thickness
A thicker, denser dermis conceals underlying contour irregularity; a thin one reveals it. Collagen loss with age and with rapid weight change is why the same anatomy becomes more visible over time.
Structure Four
Septal Orientation
Perpendicular in women, criss-cross in men. This architectural difference — not diet, not toxins, not lymphatic sluggishness — is the reason the condition is distributed the way it is across the population.

Grading

Four Grades, Three Positions

Cellulite is graded rather than described, and the grade is established by examining the same area in more than one position. What separates the grades is not how bad it looks in a photograph but whether the dimpling requires provocation to appear, appears on standing, or persists when the patient lies down. That distinction predicts what is achievable more reliably than severity of appearance does.

Cellulite is not stored toxins and it is not too much fat. It is skin being held down, which is why people lose weight and keep every dimple they started with.

Specifications

Region
Posterior and lateral thigh, buttock, occasionally the posterior arm and lower abdomen
Assessment
Graded standing at rest, standing on pinch, and lying — the three positions separate the grades
Grading
Nürnberger-Müller: Grade 0 through Grade III by whether dimpling appears on pinch, at rest, or in both positions
Review
Photographed in standardised position and lighting; reassessed at intervals of months
Grade 0–I
Provoked Only
Grade 0 shows no dimpling even when the skin is pinched. Grade I shows none at rest but reveals dimpling on pinching or on contracting the underlying muscle. The septal tethering exists but the dermis above it is still dense enough to conceal it, and the useful work here is skin quality rather than release.
Grade II
Visible Standing
Dimpling is apparent at rest when standing, and disappears or substantially softens on lying down. Gravity is loading the tethered points, which is exactly what the mechanism predicts. This is the commonest presentation at consultation and generally the most rewarding to address.
Grade III
Present Lying
Dimpling is visible both standing and lying, with palpable nodularity and often raised areas between depressions. The septae are dense and established and the dermis above them is thin. Improvement is realistic; complete resolution is not, and saying otherwise would be dishonest about what the tissue will do.

What Changes

Why Weight Loss Disappoints

The most common story in this consultation is a patient who lost a meaningful amount of weight, watched the thighs get smaller, and found the dimpling unchanged or worse. That result is exactly what the mechanism predicts. Weight loss reduces the volume of the lobules between the septae; it does nothing to the septae themselves. The skin is still anchored at the same points, and with less fat filling the compartments and a dermis that has lost support, the tethering can read more sharply than it did before.

It follows that anything which changes the depressions has to address the band that creates them. Release of the fibrous septae is the intervention that targets the actual cause, and it works on the same principle used to free tethered scars elsewhere on the body — subcision of a tethered depression releases the anchor and allows the overlying surface to rise. Applied to a cellulite dimple, releasing the band is what permits the skin to sit level again rather than merely making the surrounding area smaller.

Around that, energy-based work has a genuine but different role. Approaches that reduce localised fat volume flatten the herniation between septae, and approaches that stimulate dermal collagen thicken the covering so that residual irregularity reads less sharply — the reason radiofrequency body work and microwave-based contouring are used alongside release rather than instead of it. A plan built only on volume reduction treats one of three contributing structures. Realistic expectation is improvement in grade and in skin quality, not a surface that photographs as though the septae were never there.

FAQ

Cellulite — Common Questions

Ask a Question
No. There is no toxin, no accumulated waste and no lymphatic blockage at the root of it. Cellulite is a structural arrangement of the subcutaneous layer — fibrous septae anchoring the skin to the deeper fascia while fat lobules between them push outward into the dermis. The toxin account persists because it supports the sale of wraps, teas and drainage protocols, none of which can change the architecture that produces the appearance.
Because weight loss addresses the wrong element. It reduces the volume of fat in the compartments between the septae, but the septae themselves are unchanged and continue to tether the skin at exactly the same points. In many people the dimpling becomes more obvious rather than less, since there is less fat filling the compartments and the dermis has often thinned during the same period. This is the single most common disappointment in this area and it is entirely predictable from the anatomy.
Frequently. Cellulite is present in lean, athletic women as well as in those carrying more weight, because the determining factor is septal orientation and dermal thickness rather than fat volume. Body composition influences how prominent it appears — more fat between the septae means more herniation — but it does not determine whether the underlying architecture is there. This is also why it appears in adolescence in many women, long before any question of weight.
Septal orientation. In women, the fibrous bands run predominantly perpendicular to the skin surface, so each band pulls straight down and the fat between them can only herniate upward. In men the bands are arranged in a criss-cross lattice which distributes tension across the layer and prevents that pattern from forming. It is a difference in architecture, not in diet, discipline or hormones as such.
Not in any way that changes the structure. Topical agents cannot reach the fibrous septae, which sit well below the dermis, and no cream releases a collagen band. Vigorous massage produces temporary oedema that smooths the surface for a few hours and is routinely mistaken for a result. Agents that improve dermal hydration and collagen quality may make the skin covering slightly more forgiving, which is a marginal and temporary benefit rather than a treatment.
Improved, usually. Removed entirely, rarely, and nobody should be promised it. Release of the tethering septae changes the depressions themselves, and combining that with fat reduction and dermal collagen stimulation addresses all three contributing structures — which is as complete as this condition gets. Grade matters here: a Grade I or II presentation often improves substantially, while an established Grade III with dense septae and thin overlying dermis improves meaningfully but does not become a smooth surface. That is the honest version, and it is the one worth hearing before starting.
