Truncal Skin · Humidity · CityLink Mall
Truncal acne is not facial acne relocated. The follicles are larger, the sebaceous glands denser, the skin thicker and far more prone to raised scarring — and a substantial proportion of what presents as back acne is not acne at all.
The pilosebaceous units on the back and chest are larger than those on the face, set in a thicker dermis, and clustered at higher density. A larger unit holds more sebum and keratin behind an obstruction, so when the follicular wall gives way it releases more inflammatory material deeper into a thicker dermis. Lesions on the trunk are consequently bigger, slower and more often nodular than their facial equivalents.
Healing differs as well, and this is the more consequential point. Skin over the sternum, the shoulders and the upper back is under constant tension from movement and is the classic anatomical territory for hypertrophic and keloid scarring. The same inflammatory lesion that leaves a flat brown mark on a cheek can leave a firm raised scar on a shoulder, and unlike an atrophic scar that outcome is partly predictable from site alone.
Singapore's climate is a genuine aggravator rather than a rhetorical one. Sustained heat and humidity keep the skin surface occluded and damp, sweat and sebum are not evaporating efficiently, and clothing, backpack straps, sports equipment and car seats add friction and further occlusion. Each of these increases follicular obstruction and favours the organisms that thrive in a warm lipid-rich environment.
Which raises the diagnostic question that governs everything else. A considerable share of truncal eruptions are Malassezia folliculitis — a yeast overgrowth inside the follicle that produces uniform, itchy, comedone-free papules and does not respond to the antibacterial and comedolytic treatment acne requires. Distinguishing the two before treatment is not a formality; it changes the entire plan.
Types
Three separate conditions produce papules across the back and shoulders, and they are treated in different and partly opposing directions. Telling them apart is done on morphology, distribution and symptoms rather than on how severe the eruption appears.
If the papules on someone's back are all the same size and they itch, the question is not which acne treatment — it is whether this is acne.
Management
The practical obstacle with truncal acne is area. Treating an entire back and both shoulders with a topical agent applied twice daily is difficult to reach, expensive to sustain and poorly adhered to, which is why truncal disease is escalated to systemic therapy earlier than facial disease of equivalent severity. Medicated washes left on the skin for a minute before rinsing are a reasonable adjunct precisely because they solve the coverage problem.
In-clinic treatment on the trunk is aimed at two things: reducing surface oil and follicular obstruction across a large field, which is where a carbon laser peel has a legitimate adjunctive role, and managing what is left behind. The marks on the trunk deserve separate mention. Post-inflammatory hyperpigmentation on the back and chest routinely persists a year or longer, considerably outlasting the facial equivalent, and many patients arrive concerned about pigment rather than about active lesions.
Raised scarring is the outcome worth preventing most deliberately. Once a hypertrophic or keloid scar has formed over the sternum or shoulder, treatment is suppressive, prolonged and prone to recurrence, and the results are honestly variable. Controlling nodular truncal acne early is the only reliable way to avoid that situation.
