HIFU Complications —
When The Focus Misses

HIFU can go wrong in four recognised ways: nerve irritation, fat loss, surface burns and pain over bone. This page explains how each one happens, what Dr Sin Yong looks for when you come to Atelier after a problem, what can and cannot be corrected, and which signs need a doctor the same day.

The Mechanism

When Heat Reaches The Wrong Layer

Book a Consultation

HIFU converges ultrasound into small points of heat, around 60–70 °C, at fixed depths of 1.5, 3.0 or 4.5 mm below the surface, leaving the skin on top largely untouched. How each zone is mapped before treatment is set out on the HIFU treatment page.

The cartridge fixes the depth, but the tissue beneath it changes from zone to zone. Where the face is thin, or a line crosses a nerve, a fat pad or bone, the heat lands somewhere it was never aimed. Every problem on this page comes back to depth, placement or technique.

Layer One
Nerve Branches
Two facial nerve branches run near treatment depth where tissue is thin: the marginal mandibular branch along the jaw border, and the temporal branch above the brow.
Layer Two
Fat
The 4.5 mm cartridge is built to reach the SMAS. Over thinner tissue, the focus sits in the fat under the skin instead, and coagulated fat does not grow back.
Layer Three
Skin Surface
A tilted transducer, too little gel, or a cartridge too shallow for the skin leaves energy at the surface. Stacked passes along one line concentrate it further.
Layer Four
Bone
The periosteum, the membrane over bone, is rich in nerves. Over the jawline, cheekbone and forehead it can sit close to the focal depth.

Types

Four Problems, Four Pictures

Some redness, swelling and tenderness is expected after any HIFU session. The four problems below fall outside that. If your concern is that little changed, that is a different question, covered in why HIFU or Ultherapy results can disappoint.

Specifications

Region
Jaw border, cheek, temple, brow and forehead, where tissue over nerve or bone is thin
Assessment
Examination, with the date, device and areas from your treatment record
Modalities
Follow-up for nerve symptoms; sun protection and treatment to limit scarring for burns; filler or a collagen biostimulator for settled hollowing
Review
Same day for the warning signs below; hollowing once the tissue has settled
Type I
Nerve Weakness Or Numbness
Numbness, tingling, or the corner of the mouth or one brow moving less than the other side. Published case series describe these symptoms as temporary, but no one can promise how long recovery will take.
Type II
Fat Loss And Hollowing
A new hollow in the cheek or temple. The risk is higher in lean or volume-depleted faces, in older faces whose fat pads have thinned, and where a preset plan replaced palpation.
Type III
Burns, Welts And Blisters
A raised line along the path of the transducer. Most settle as swelling goes down. One that stays, blisters, or turns dusky or dark needs a doctor; in Fitzpatrick III to V skin a burn can pigment or scar.
Type IV
Bone Pain And Swelling
A deep ache over bone during treatment is usual, as is jaw tenderness for a short while after. Pain that is severe, lasts past the early days, or arrives with marked or one-sided swelling is not.

The Decision

Why More Energy Is Not The Fix

At Atelier, Dr SY starts with what was done: the date, the device, the areas treated and any record you were given. He then examines the area to name which of the four problems it is, because each runs its own course.

One rule holds for all of them: a HIFU complication is not treated with more HIFU. Another pass of focused heat does not undo an irritated nerve, a burn or a lost fat pad, and over fat it only adds to the loss.

Volume work, where needed, is planned from the examination rather than offered on the day. The options at Atelier are measured volume restoration with hyaluronic acid filler, including cheek and temple filler, or a collagen biostimulator. Darkening left by a burn is post-inflammatory hyperpigmentation, which has its own page.

Safety

Warning signs: when to get help now

Redness and tingling for hours, and tenderness for a few days, are expected after HIFU. The signs below are not. Act on them the same day rather than waiting to see.

  1. Same day: contact the clinic that treated you, or another doctor, if you notice any change in facial movement, such as a mouth corner or brow moving less, blistering, a raised line that will not flatten, swelling that is marked or on one side only, or numbness that is not easing.
  2. Tell them what was done: when, which areas and on which device. Keep any treatment record you were given.
  3. For a burn, welt or blister: leave the area bare and out of the sun until a doctor has seen it.
  4. For bone pain that is severe, lasting or swollen: have it examined rather than relying on painkillers alone.
  5. Do not book more HIFU over the area to put it right.

FAQ

HIFU Complications — Common Questions

Ask a Question
Published case reports describe nerve symptoms after focused ultrasound as temporary, settling as the irritation of the nerve branch eases. Even so, nobody can give you a recovery date in advance. Any change in how your mouth or brow moves should be examined and recorded promptly, and weakness that persists is followed up rather than assumed to be resolving.
Fat coagulated by focused heat does not regrow, so waiting will not fill the hollow. Once the tissue has settled, Dr SY examines the area and decides whether volume restoration with hyaluronic acid filler or a collagen biostimulator suits it, and how much. The plan comes from that examination, so it is not offered automatically at your first visit.
Not necessarily. Most raised lines after HIFU go down as the swelling settles. A line that stays, blisters, or turns dusky or dark is different and needs to be seen by a doctor. Until then, leave the area bare and keep it out of the sun. Surface burns can scar or leave pigmentation, and that risk is higher in Fitzpatrick III to V skin.
A deep ache or a brief jolt over bone during treatment is the usual sensation, because the membrane covering bone is sensitive and sits close to the focal depth where tissue is thin. Tenderness along the jaw for a few days is expected. Pain that is severe, keeps going past the early days, or comes with marked or one-sided swelling should be examined.
It depends on what caused the problem. When thin tissue was the reason, as with fat loss in a lean face, a different mechanism usually suits better than another round of focused ultrasound. When technique was the cause in a face that suits HIFU, repeating it later can be reasonable, but only after a fresh assessment and a new depth map of each zone.
Where tissue over a nerve, a fat pad or bone is thin. The jaw border carries the marginal mandibular branch, and the area above the brow the temporal branch. The cheek and temple can lose fat when a deep cartridge meets thin tissue, and the jawline, cheekbone and forehead lie close to bone. Lean or volume-depleted faces carry more risk throughout, which is why each zone is mapped first.
