Who Is Thermage Suitable For? —
Skin First, Then Structure

Thermage suits skin that has loosened while the face beneath it still holds its volume. This page sets out which findings make Thermage FLX a good fit, which point elsewhere, and who should not have it, as Dr Sin Yong, Global Key Opinion Leader in Facelifts & Body Contouring, assesses it at Atelier.

Short Answer

Thermage Suitability, In Brief

Book a Consultation

Thermage FLX tends to suit mild to moderate laxity of the skin itself: a jawline or cheek that has softened, with volume still in place and skin that springs back when gently pinched. It suits deeper descent, lost volume or heavy fat poorly, and anyone with a pacemaker or other implanted electronic device should not have it.

Which Layer

Loose Skin Or Tissue That Has Slipped?

Thermage passes monopolar radiofrequency at 6.78 MHz into the dermis and the fibrous strands below it while cryogen cools the surface. The collagen there contracts and then remodels. It does not reach the SMAS, the deeper support layer, and it adds no volume and removes no fat or skin.

So the first task at consultation is to work out which of three changes is driving the look: laxity of the skin, descent of deeper tissue, or loss of volume. A jawline just beginning to soften is the classic Thermage finding. A jowl that hangs in a pocket, or a midface that has slid, sits in a layer Thermage does not reach; heating skin over descent, fat or hollowing is not a plan.

Safety

Who Should Not Have It, And Who Should Wait

The current travels from the treatment tip through the body to a return pad, which is why the exclusions are firm.

  • Not suitable: a pacemaker, defibrillator or any other implanted electronic device.
  • Wait: pregnancy, and skin that is infected, broken or inflamed in the area, including an active cold sore or eczema flare, which is treated first.
  • Changes the plan: metal implants or plates near the area, because current gathers around metal; recent filler or threads, which are mapped and avoided; conditions that flare with heat; slow wound healing; and reduced sensation, since energy is guided by what you feel.

Skin colour is not a barrier. Radiofrequency heats through the tissue's electrical resistance rather than through melanin, so darker Asian skin is assessed in the same way. What can follow if something does go wrong is on Thermage side effects.

How This Is Assessed At Atelier

Dr SY examines the face at rest and in movement, pinches the skin to see how well it recoils, and asks about implants, metalwork, earlier filler, threads and energy treatments.

If Thermage fits, the plan names the areas and how each will be treated; the treatment itself is described on the Thermage page. If it does not, the written plan names what would suit better, and choosing no treatment is always open to you. For how radiofrequency compares with focused ultrasound, read HIFU vs Thermage vs Volnewmer.

FAQ

Thermage Suitability — Common Questions

Ask a Question
Thermage FLX tends to suit adults with mild to moderate looseness of the skin, where facial volume is still in place and the skin recoils when pinched. It is a poorer fit for a hanging jowl, a deflated midface, heavy fat or large excess skin, each of which needs a different approach. Anyone with an implanted electronic device should not have it. Dr SY confirms suitability after examining you.
Often, that is the stage Thermage suits. Softness along the jaw with no hanging jowl usually reflects a change in the skin envelope. Once a jowl hangs in a distinct pocket, the deeper tissue has moved, and focused ultrasound or another lifting plan is considered instead. The change builds over two to six months.
A pacemaker, defibrillator or other implanted electronic device rules Thermage out, because the current passes through the body to a return pad. Metal implants, such as plates or screws near the treatment area, do not always exclude it, but they change the plan, since current concentrates around metal and that zone may be left untreated. Tell the clinic about any implant before your consultation.
It needs caution. Hollowing after energy-based tightening is a recognised effect, and a lean face has little fat to spare. Tightening skin over an area that is already emptying can make it look more hollow. In a slim face Dr SY may suggest restoring volume first, a lighter radiofrequency plan, or no tightening at all, depending on where the laxity sits.
