Pico Laser for Acne Scars —
Marks And Shallow Texture

Pico laser for acne scars does two separate jobs at Atelier: low-energy toning for flat brown marks once acne is under control, and fractional delivery for shallow texture. Dr Sin Yong sorts every mark and scar by type first, because pico laser cannot lift a tethered scar, reach an ice-pick tract or fade a red mark.

The Mechanism

Two Modes, Two Targets

Book a Consultation

Toning sends low-energy picosecond pulses across a brown mark. They fragment the melanin left behind by inflammation, and the skin clears the fragments over the following weeks.

Fractional delivery focuses the beam into a grid of tiny injury zones beneath a largely intact surface. The skin repairs each zone with new collagen, which softens shallow irregularity. Because little heat spreads, recovery is lighter than with ablative lasers and skin of Fitzpatrick III to V carries a lower, though not zero, pigment risk.

Who It Suits

Mark Or Scar? Sorting Comes First

Dr SY stretches the skin and looks at it under angled light. Colour that vanishes on stretching is a mark; a dent that stays is a scar; a dent that dimples and deepens under the stretch is held down from beneath. Red or pink marks are dilated vessels, and a picosecond pulse does not target haemoglobin; see red and brown acne marks and vascular laser. Scar shapes are explained on acne scar types.

The Decision

Where Pico Sits In Acne Scars Treatment

The usual order at Atelier is: settle active acne (acne treatment), release tethered scars with subcision, treat ice-pick tracts with TCA CROSS, resurface deeper edges with fractional CO2 (DEKA Tetra Pro) or RF microneedling, then use pico for the remaining colour and fine texture. Many faces need only some steps.

Fractional pico and CO2 are not ranked; they do different jobs. Which Atelier laser suits which scar is set out in acne scar laser treatment.

Pico Laser

On The Day

A Fractional Session, Step By Step

  1. The skin is checked for active breakouts, a recent tan or irritation, any of which postpones treatment.
  2. Numbing cream is applied, then removed, and eye protection goes on.
  3. Dr SY treats the area with the DEKA TORO's fractional mode, with density and energy set conservatively for darker skin; a small test area may come first.
  4. The skin is cooled and sunscreen applied; written aftercare follows.

Afterwards

The Days That Follow

Expect pinpoint redness, mild swelling and a sandpapery feel, then fine flaking. Day-by-day timings are on pico laser downtime and home care on pico laser aftercare. Daily sun protection runs through the whole plan; a topical retinoid or other prescription cream may be discussed, and whether it suits you is decided at a consultation.

Risks

What Can Go Wrong

Fractional work can bring post-inflammatory darkening, pinpoint bleeding, redness that lingers, an acne flare or a cold sore around the mouth. Toning repeated too often can leave pale, mottled spots. Darker skin, sun on healing skin and picking at flakes all raise the chance of brown marks.

Contact the clinic the same day for pain or swelling that is increasing, pus, or skin that turns dusky.

FAQ

Pico Laser for Acne Scars — Common Questions

Ask a Question
Neither is better in general; they reach different depths. Fractional pico works beneath an intact surface with little heat, so recovery is lighter and the pigment risk is lower, which suits shallow texture and fine rolling scars. Fractional CO2 removes columns of skin and heats the dermis around them, so it reaches deeper boxcar edges, with more crusting and a longer recovery. Many plans use both, at different stages.
Not for scars or marks. Treatment waits until active acne is controlled, because each new spot can leave a fresh mark beside the one being treated, and laser over inflamed skin adds to the problem. Acne treatment comes first. Once breakouts have settled, Dr SY types each mark and scar and plans the order.
Not on its own. An ice-pick scar is a narrow, deep tract, and fractional pico does not reach its base. At Atelier these are usually treated first with TCA CROSS, which works inside each tract. Fractional pico or another resurfacing step may follow later to even out the surrounding surface.
Because the dents are probably not a pigment problem. Toning lightens colour; it does not lift a scar. If the same depressions remain after several rounds, they are usually tethered, boxcar or ice-pick scars that need subcision, TCA CROSS, fractional CO2 or RF microneedling. A fresh assessment that sorts each scar by type is the useful next step.
Most people see pinpoint redness and mild swelling at first, then a dry, sandpapery texture and fine flaking as the micro-zones heal. A spot that turns darker weeks later is not part of normal healing: it should be looked at by Dr SY rather than treated over at the next session.
