Boxcar Scars Treatment —
Measured By The Shadow

Boxcar scars treatment depends, more than on anything else, on one measurement: the depth of the box. At Atelier, Dr Sin Yong judges each box scar under side light, then matches shallow ones to the non-ablative FSX Laser, deep ones to fractional CO2, and narrow corners to TCA CROSS.

The Mechanism

Steep Walls, A Flat Floor

Book a Consultation

A boxcar scar is a round or oval dip with near-vertical walls and a flat base, shaped like a U in cross-section. It forms where inflammation destroyed collagen in the dermis, and it ranges from a shallow dish to a deep well. Most cheeks with boxcar acne scars carry other types too, which is why the whole face is mapped, using the groups set out in acne scar types.

Depth is read from the shadow. Under side light, a steep wall throws a long shadow. Stretching the skin adds a second clue: a box that stays dented has a depth problem, while one that flattens is being held down from below and is treated like a rolling scar. A narrow, deep corner behaves like an ice-pick tract.

Box By Box

Shallow Box, Deep Box, Narrow Corner

Each box scar is sorted by what the examination shows, and the method follows. In Fitzpatrick III to V skin, the gentler option is used wherever it reaches deep enough.

Boxcar types and how each is usually treated
What the examination showsUsually treated with
Short walls, short shadow, stays dented on stretchFSX Laser, or fractional CO2 at a light setting beside deeper scars
Steep walls, long shadow, still a well on stretchFractional CO2: point shots into each box, then lighter passes so the edges blend
One narrow, deep cornerTCA CROSS into the corner, then laser to even the surrounding surface
Flattens when the skin is stretchedSubcision first, then resurfacing
Shallow to moderate boxes in darker skinRF microneedling with insulated needles, or FSX
A small, isolated, very deep boxAssessed at Atelier; a punch technique arranged by referral where needed

The Decision

Choosing The Step, And When To Wait

Fractional CO2 changes more texture per session than a non-ablative laser, but healing takes longer and the risk of darkening is higher in Fitzpatrick III to V skin, so it is kept for boxes that need it. See Tetra Pro Scar³ and acne scar laser treatment.

Wait first if acne is still active, because new spots make new boxes. A fresh tan, recent isotretinoin, a history of cold sores or a tendency to keloids changes the plan. Punch excision and punch elevation are surgical; they are discussed at Atelier and arranged by referral to a plastic surgeon where needed.

On The Day

What A Treatment Visit Involves

  1. Your scars are graded box by box, and photographs are taken with consent for your medical record only.
  2. A written plan lists the step for each box, and the order.
  3. After numbing cream, the planned step is done and you leave with written aftercare.

Recovery

Healing Between Stages

After FSX, redness settles and fine scabs are left to shed. After fractional CO2, expect redness, swelling and warmth, then fine scabbing and peeling while the columns close; strict sun protection follows. See CO2 laser aftercare, TCA CROSS aftercare and acne scar treatment downtime.

Risks

Risks And Side Effects

Less common risks: prolonged redness, darker or paler patches, infection, a cold sore flare and, rarely, a new scar. No outcome is promised. Contact the clinic the same day if pain, redness or swelling increases, or pus or blisters appear.

FAQ

Boxcar Scars — Common Questions

Ask a Question
Shallow ones often can. Boxes with short walls usually respond to the non-ablative FSX Laser, and in darker skin RF microneedling is an option for shallow to moderate boxes. Deep boxes with steep walls are different: a non-ablative laser works in the upper dermis and tends to under-treat them, so they usually need fractional CO2.
Not reliably. Front lighting and phone cameras hide the shadow of a scar wall. Depth is judged in person under side light, with the skin stretched to see whether the box stays dented or flattens. A photo is useful for booking, and the decision on which step suits each scar is made at the examination.
TCA CROSS places a tiny amount of strong acid into a narrow, deep space so that it rebuilds from the base. It suits a box with a narrow corner that behaves like an ice-pick tract. A wide, shallow box has no narrow tract to fill, so acid adds little there and laser is used instead. See TCA CROSS for acne scars for how the acid is placed.
When a single box is small but too deep for acid or resurfacing to change much. Punch excision swaps the box for a fine stitched line, and punch elevation lifts its base to skin level. Both are surgical: Dr Sin Yong assesses whether one would help, and the procedure is arranged by referral where needed, with laser planned around it.
On shallow to moderate boxes and the texture around them, yes, and it is often chosen in darker skin because the insulated needles leave the surface largely untouched. It does not vaporise steep walls or reach a narrow, deep corner, so deep boxes usually need fractional CO2 or TCA CROSS as well. RF microneedling for acne scars covers the device in detail.
