Rolling Scars Treatment —
Release, Support, Then Resurface

Rolling scars treatment has to begin beneath the skin, because a rolling acne scar is held down by fibrous bands rather than missing from the surface. At Atelier, Dr Sin Yong releases the bands with subcision, adds support only where the floor has thinned, and then chooses RF microneedling or fractional CO2 for what remains on the surface.

The Mechanism

Good Skin, Pinned Too Low

Book a Consultation

A rolling scar is a broad dip, usually wider than four to five millimetres, with soft sloping shoulders and a wavy surface. Deep acne inflammation healed with fibrous bands that tie the dermis to the tissue beneath, so ordinary skin cannot sit level with its neighbours.

Three signs point to a tether. The dip looks worse in light from the side, it deepens when you smile, and it flattens or nearly vanishes when the skin around it is stretched. A dent that keeps its sharp edge under stretch is a different scar, covered on boxcar scars treatment. Many cheeks have both.

Specifications

Target
Rolling acne scars tethered by fibrous bands
Method
Subcision first; support where the floor has thinned; then RF microneedling or fractional CO2 according to depth
Sessions
Subcision typically 2–4 sessions, 6–8 weeks apart; resurfacing scheduled once bruising has settled
Downtime
Bruising and swelling for 5–10 days after subcision; resurfacing has its own recovery
Numbing
Local anaesthetic for subcision; numbing cream for resurfacing

Three Steps

Rolling Scars Treatment: What Each Step Does

Each step does one job. Done in order they build on each other; reversed, they work against each other.

The steps in a rolling scar plan and their limits
StepWhat it doesWhat it cannot do
SubcisionCuts the band through a small puncture, so the skin can rise; new collagen forms in the spaceSmooth texture, soften edges or reach an ice-pick tract
SupportA collagen stimulator placed in the released space where the floor has thinnedLift a scar while the band is still intact
RF microneedlingHeats the dermis at a set depth with the surface largely spared; suits shallower scars and darker skinCut a band
Fractional CO2Softens steep edges on deeper scarsRelease a tether or replace lost volume
Laser alone, no releaseImproves the surface around the scarChange the depth for long: better skin, same dimples

The Decision

Why Energy Alone Stalls, And Who Should Wait

Lasers and radiofrequency act from above, and the band sits deeper than resurfacing energy is meant to reach. More energy does not cut it, and in Fitzpatrick III to V skin it adds the risk of darkening. Why rolling scars plateau after laser is explained in subcision vs laser for rolling scars.

Wait first if acne is active, if you have taken isotretinoin recently, or if you are pregnant. Blood-thinning medicines or a bleeding tendency matter for subcision; a prescribed blood thinner is never stopped without the prescriber's advice. A keloid tendency is weighed carefully. A scar too deep or wide for these methods is assessed at Atelier, and a surgical punch technique arranged by referral where needed.

Subcision For Tethered Scars

On The Day

A Subcision Visit

  1. Tethered scars are marked under side light and stretch.
  2. After local anaesthetic, a fine needle or blunt cannula releases each band through a small entry point.
  3. Support goes in only where planned, and you leave with written aftercare.

Details are on subcision for tethered scars.

Recovery

Recovery Between Steps

Bruising, swelling and tenderness are expected and settle; cold compresses help, and strenuous exercise waits until the swelling eases. Resurfacing follows once bruising has fully gone, and in darker skin staging is often more conservative. See acne scar treatment downtime.

Risks

Risks And Side Effects

Bruising, small firm lumps, infection, pigment change and less improvement than hoped are possible. A band can partly re-form as it heals, which is why release is planned as a staged course. No outcome is promised. Contact the clinic the same day if swelling, pain or redness increases, or an entry point discharges pus.

FAQ

Rolling Scars — Common Questions

Ask a Question
By cutting the fibrous band that holds it down. Through a small puncture beside the scar, a fine needle or blunt cannula is passed beneath it and swept until the band gives way, which is often felt as a sudden release. The skin can then rise, and over the following weeks new collagen forms in the space underneath, helping to hold it there.
Because the body heals a cut band, and sometimes the healing re-forms part of it. Deep, long-standing bands are also less likely to release fully in one pass. For both reasons subcision is planned as a staged course rather than a single visit, with each scar checked before the next release.
Only where a released scar is also short of volume. Filler placed under a band that is still intact tends to spread around the scar instead of lifting it, so release comes first. Where the floor has thinned, a collagen stimulator can be placed in the released space; where it has not, nothing is added. The decision is made scar by scar.
RF microneedling, which heats the dermis through insulated needles and largely spares the surface, usually suits shallower rolling scars and darker skin. Fractional CO2 suits deeper scars with steep edges. For fine texture between scars, the non-ablative FSX Laser has a shorter recovery. Acne scar laser treatment explains how the lasers differ.
Sometimes. As the midface loses volume with age, the support beneath the cheeks drops and every dip on it casts a deeper shadow, so scars can seem to worsen while the scars themselves are unchanged. That is a separate problem with a separate treatment, and it is told apart by examination rather than by photograph. Both can be planned for together.
