HIFU Treatment for Brow and Eyelid Heaviness —
Stopping At The Rim

HIFU treatment for brow and eyelid heaviness is delivered to the forehead, brow and temple, never to the eyelid itself. It can help when a lowered brow is pressing on otherwise normal lids; it does not correct a weak eyelid muscle or remove loose lid skin.

The Mechanism

Three Kinds Of Heavy Eyelid

Book a Consultation

A heavy upper lid has one of three causes, or a mix. The brow may have descended onto normal lids. The lid skin may be loose and fold over an intact crease (dermatochalasis). Or the levator, the muscle that raises the lid, may be weak, so the lid margin sits low (ptosis). Droopy and hooded eyelids explains each one.

HIFU works on the first only. Shallow points of heat in the forehead and brow remodel over 8–12 weeks, firming the tissue that has let the brow settle. The lid skin and the levator are not reached.

At A Glance

Shallow Settings, Stopping At The Bone

Dr Sin Yong, Global Key Opinion Leader in Facelifts & Body Contouring, uses only shallow transducers here, because the tissue is thin and close to bone; the 3.0 mm and 4.5 mm depths used on the jaw are left out. Lines run across the forehead and along the upper brow border and stop at the bony rim around the eye, so no focal point is aimed at the eye or lid. Placement respects the nerve to forehead skin and the branch that raises the brow.

Who It Suits

When HIFU Treatment Suits A Heavy Brow

HIFU suits heaviness driven by the brow: an intact double-eyelid crease, a lid margin at normal height, and a brow that lifts most of the heaviness away when nudged up with a fingertip. Nothing is cut and nothing touches the lid, so your crease keeps its shape.

The Decision

When The Answer Is Something Else

  • Ptosis: the levator needs surgical correction, which no energy device reaches; Atelier refers.
  • Loose lid skin resting on the lashes: referral to a plastic or oculoplastic surgeon for blepharoplasty assessment.
  • A brow pulled down by muscle: small doses of BTX in the brow-depressing muscles can let it sit a little higher.
  • A hollow temple under the brow tail: temple fillers restore the support the brow rests on.
  • Loosened support around the eye: the VF Lift – Vertical Facelift includes the brow as a radiofrequency zone, and Ultherapy shows the layers before energy is placed.

How these combine is on the non-surgical eyelid lift page.

HIFU Focused Ultrasound Lifting

On The Day

A Brow HIFU Appointment

  1. Measurements and photographs with your consent: lid margin height, brow position against the bone, and the lids with the forehead relaxed.
  2. The fingertip brow test, to confirm the brow is the cause.
  3. Numbing cream, removed before treatment.
  4. Lines marked across the forehead and above the brow, ending at the rim.
  5. Shallow lines delivered, with energy adjusted to what you feel.

Afterwards

Recovery And Aftercare

Redness, warmth and brow tenderness settle on their own, and the upper lids may look slightly puffy the next morning; see HIFU downtime and HIFU aftercare. Any change in brow position is gradual and judged at about three months.

Safety

Risks Around The Eye

  • Heat near the nerve above the eye can leave a tender or numb forehead patch, or a headache, which settles.
  • If heat reaches the temporal branch of the facial nerve, one side of the forehead can be weak for a while, so that brow will not raise.
  • In a lean temple, energy can leave a hollow.

Contact the clinic the same day if a brow will not raise, numbness persists, or swelling is marked or one-sided. A change in vision or eye pain should not wait for a reply: call 995 or go to the nearest hospital emergency department. HIFU complications explains the signs.

FAQ

HIFU for Brow and Eyelid — Common Questions

Ask a Question
No. Lines cover the forehead, brow and, where suitable, the temple, and stop at the bony rim around the eye. Focused ultrasound concentrates heat at a point beneath the skin, so nothing is aimed at the eye or the thin lid skin. Crepey or loose lid skin is assessed separately.
It should not, because no energy reaches the lid and nothing is cut. If the brow sits a little higher afterwards, more of your existing crease may show, but the crease keeps its shape. Creating or deepening a crease is a surgical question, which is discussed and referred rather than treated with HIFU.
It can happen briefly, because the nerve supplying forehead sensation rises from the rim above the eye, near the brow lines. A mild headache or tender, numb patch usually settles. Numbness that persists, or a brow that will not lift when you raise your eyebrows, should be reported the same day.
Modestly. In a rater-blinded study of ultrasound tightening cited on Atelier's HIFU page, the average brow lift measured on photographs was under 2 mm, building over weeks. That can ease mild, brow-driven heaviness but will not remove a fold of excess skin. The result is judged at about three months.
It needs care. Temple lines use shallow settings and lower energy in a lean face, because heat there can deepen a hollow. If the brow tail dropped because the temple lost volume beneath it, temple filler often helps more than further energy, and Dr SY may suggest it first.
