Ellansé Singapore —
Dosed For The Collagen, Not The Gel

Ellansé in Singapore is a polycaprolactone (PCL) filler that works in two stages: a gel gives shape on the day, then your own collagen forms around tiny microspheres over the following months. At Atelier it is one of the structural materials used for volume restoration, and Dr Sin Yong places it conservatively because, unlike hyaluronic acid, it cannot be dissolved.

The Mechanism

A Gel That Leaves A Scaffold

Book a Consultation

Ellansé is made of polycaprolactone microspheres, roughly 25 to 50 micrometres across, suspended in a carboxymethylcellulose gel. By composition it is about 30% microspheres and 70% gel.

On the day, the gel gives immediate volume, so the treatment behaves like a filler. Over the following weeks the gel is absorbed, and fibroblasts lay down collagen around the microspheres left behind, which act as a scaffold. The microspheres themselves break down slowly, eventually to carbon dioxide and water. What lasts is the collagen your body built while they were there.

That sequence changes how it is dosed. The look on the day is not the final result, and the collagen that follows differs from it in shape and amount. Overfilling on day one tends to produce a heavier face months later, with no enzyme to reverse it. So the Ellansé dose is planned around the collagen it will build, not the gel you see in the mirror.

It comes in several formulations, which differ in how slowly the body resorbs the microspheres. It is CE-marked in Europe and is not FDA-approved in the United States. In Singapore, dermal fillers are regulated by the Health Sciences Authority as medical devices.

Ellansé: The Plan In Brief

The details below describe a typical plan; yours follows an examination. Product choice across the face is set out on volume restoration.

Specifications

Target
Structural volume loss: chin projection, jawline definition, hollow temples and lateral cheeks, backs of the hands
Method
Polycaprolactone microspheres in a gel, placed deep on bone or in deep fat in small, deliberate deposits
Sessions
Assessed at consultation; staged, with a conservative first placement judged once the gel has gone and collagen has begun to form
Downtime
Swelling, tenderness, possible bruising and temporary firmness; after injectables these usually settle within 3 to 7 days
Numbing
Topical anaesthetic first, with lidocaine mixed in or given alongside

Who It Suits

Ellansé Singapore: Who It Suits, And Who It Does Not

Ellansé is a structural material. It tends to suit:

  • a chin that lacks projection, or a jaw that lacks definition, often planned together (see jawline filler);
  • hollow temples and a flattened lateral cheek, where support is read before anything goes lower in the face;
  • the backs of the hands, where tendons and veins have become prominent (see hand rejuvenation);
  • someone who understands that the result cannot be dissolved and prefers a change that builds from their own collagen.

It does not suit the lips, the tear troughs or fine lines in thin, mobile skin, where firm particles can be felt or seen. It is not a first injectable for a face that has not been assessed, and it does not lift. Volume added below a descended jowl makes the jowl heavier; that problem is assessed for HIFU or the VF Lift – Vertical Facelift instead. Dr Sin Yong holds a Key Opinion Leader appointment with CLASSYS, which makes the Volnewmer platform used for the VF Lift.

The Decision

Ellansé, Another Material, Or Nothing Yet

Hyaluronic acid if reversibility matters, if this is your first filler, or if the area is the lips or under the eyes. Radiesse (calcium hydroxylapatite) does a similar structural job with a different material and tempo, and is more often diluted for skin quality. Poly-L-lactic acid gives almost no volume on the day and suits broad, even deflation. These are compared in Radiesse vs Ellansé vs HA filler and, more generally, on collagen biostimulators.

Subtraction before addition. Heaviness or lumps from earlier injections usually need product taken away, not more added. Ellansé is not placed over earlier filler that has not been identified, or over silicone or other non-absorbable material. Where the history is unclear, imaging such as ultrasound shows what is there, how deep, and whether a lump is old filler, a cyst or scar. If earlier filler has left the face heavy, filler correction comes first.

Do not have Ellansé with an allergy to its components or a history of severe allergy, an autoimmune disease or immune-modifying treatment, a bleeding disorder, during pregnancy or breastfeeding, or with infection or inflammation near the site, such as active acne or a cold sore. Timing is adjusted for blood thinners, a tendency to keloid or thick scars, and recent or planned dental work. Suitability is confirmed at consultation.

Volume Restoration

On The Day

Planning Visit, Then Placing Visit

  1. A planning visit comes first. Dr SY reads the face as structure, the jaw from chin to angle and the temple and lateral cheek before the lower face, and asks about every earlier injectable with dates. Photographs are taken with your consent for your medical record only.
  2. You receive a written plan naming the zones, the formulation and the amount, and time to consider it.
  3. On the placing visit the skin is cleaned and marked, and a topical anaesthetic applied.
  4. Ellansé goes in deep, in small deliberate deposits rather than a sheet, with the firmer formulation used sparingly. The first placement is deliberately conservative.
  5. Skin colour over the treated areas is checked before you leave, and you receive written aftercare.
  6. A follow-up visit is timed for when the gel has gone and collagen has started to form, the first point at which the real result can be judged. Any further placement is decided then.

Recovery

Leave It To Settle

Expect swelling, tenderness, possible bruising and a temporary firmness where the gel sits. The area may look slightly less full as the gel is absorbed over the following weeks; that is the gel going, not the treatment failing. Leave the area alone: no massage or pressing unless told to.

Phase-by-phase care is on Ellansé aftercare, and how you are likely to look day by day is on Ellansé downtime. No duration is promised: the microspheres are resorbed over time, and the collagen they prompted ages with the rest of your face.

Risks

Risks And Side Effects

Common: swelling, bruising, tenderness and temporary firmness, which settle on their own.

Specific to firm, non-dissolvable products: nodules, and product that can be felt or seen. Nodules are described with all collagen stimulators. A lump that grows, hardens, becomes red, hot or painful, even long after treatment, needs examination rather than massage. After imaging, inflammatory nodules are treated medically and other lumps are managed case by case; hyaluronidase has no effect on Ellansé or its gel. See dermal filler complications.

Rare and serious: a blocked blood vessel, which depends on depth, tissue plane and anatomy. Skin that turns white, dusky or mottled, or increasing pain, needs same-day assessment; any change in vision is an emergency, so call 995 or go to the nearest hospital emergency department. See vascular occlusion warning signs.

FAQ

Ellansé — Common Questions

Ask a Question
It is both, one after the other. The gel carrier gives volume straight away, much like a filler. As the gel is absorbed over the following weeks, the polycaprolactone microspheres stay behind and your fibroblasts build collagen around them. The lasting result is that collagen, which is why the plan is dosed for what will form rather than for how the area looks on the day.
Because those are the facts for this product: it carries a CE mark as a medical device in Europe and does not have US FDA approval. In Singapore, dermal fillers are regulated by the Health Sciences Authority as medical devices. Whatever product is used, make sure its name is recorded in your notes.
No. Adding a firm, non-dissolvable product to a face that is already heavy or lumpy usually makes the problem harder to fix. The first step is to find out what is there, with an examination and imaging where the history is unclear. Excess hyaluronic acid can then be dissolved and the face left to settle before deciding whether any structure is still missing.
No. Ellansé adds structure and collagen, but it does not move tissue that has slipped. Placing volume below a descended jowl makes the lower face heavier. Where the jowl comes from laxity, lifting such as HIFU or the VF Lift – Vertical Facelift is assessed first, and structure along the chin or jaw may be considered afterwards if it is still needed.
Fees for medical treatment are not advertised in Singapore, and a figure given before an examination would only be a guess. What changes the quote is which areas are treated, the formulation, the amount needed, how placement is staged and whether it is combined with other treatment. The consultation also decides whether Ellansé is advised at all. You receive a written quote afterwards; How We Quote explains what goes into it.
Because the final result is not visible on the day. The gel fades over weeks and the collagen builds over months, so a face that looks right at the end of the appointment may look heavier later, with no way to dissolve the excess. A smaller first placement, judged once the gel has gone, lets Dr SY see how your tissue responds before deciding whether more is needed.
