Dermal Fillers Singapore vs BTX —
How They Differ

Dermal fillers in Singapore are often spoken of in the same breath as BTX, because both come in a syringe and both are aimed at lines and shadows. They do different jobs: botulinum toxin quietens a muscle, while filler occupies space that has been lost. This guide helps you tell which problem you have before you book.

How Each Works

One Acts On Muscle, The Other On Space

BTX is botulinum toxin, a prescription medicine injected in small doses into a chosen muscle. It blocks the chemical signal that tells the muscle to contract, so the skin above it folds less often and less deeply. At Atelier the change starts at 3–7 days, reaches its full effect at about 14 days, is checked at two weeks and lasts around 3–4 months. More is on BTX for wrinkles and prevention.

Dermal filler is a gel placed in a chosen tissue plane to restore volume or add projection. Most filler at Atelier is hyaluronic acid; calcium hydroxylapatite and polycaprolactone are also used where firmer, collagen-stimulating support suits. The visible effect lasts roughly 6–18 months, depending on product and placement. How product is matched to depth is on volume restoration.

So filler does not relax a muscle, BTX adds no volume, and neither tightens loose skin, lifts descended tissue or improves skin quality.

Which Problem

Which Line Needs Which

Look in the mirror with your face still. A line that vanishes at rest is mostly muscle; a fold or hollow that stays is usually volume, sometimes with the skin involved.

  • Frown lines, forehead lines and crow's feet: BTX. If a frown line has become etched into the skin at rest, BTX stops the folding but does not fill the groove. Skin treatment may follow, and filler is used there only in small amounts and with great care, because the vessels between the brows carry a higher risk.
  • Nasolabial folds: not a muscle problem. They deepen as the cheek above deflates and descends, so support is restored at the cheek first and the fold itself is treated last, if at all.
  • Downturned mouth corners: often both. A small dose of BTX eases the muscle that pulls the corners down, and filler or a biostimulator supports the chin and the area in front of the jowl. See smile and marionette lines.
  • Hollow cheeks, flat temples, a short chin: filler alone; BTX has nothing to offer here.
  • A wide jaw from a bulky masseter: BTX reduces the muscle (jaw BTX); a little chin filler sometimes balances the lower face.

Both In One Plan

When Both Are Planned

Many faces past their mid-thirties show movement and volume change at once, and the lower face is the commonest example. When both treatments are aimed at the same area, BTX usually goes first. A relaxed muscle changes how much volume the area seems to need, so the face is seen again at the two-week check before any filler is decided.

The upper face needs the same caution. A forehead that works hard to hold up heavy brows is examined before anything is injected, because relaxing it fully can let the brows drop. Restoring a hollow temple can also change how the brow sits before any muscle is touched.

Combining is not doing more: each product is dosed to the examination, and the face should still move. Where a bioremodeller fits alongside both is set out in Profhilo vs fillers.

Reversal And Risk

What Happens If It Is Not Right

The two fail in different ways. Too much BTX, or BTX that reaches a neighbouring muscle, can leave a heavy brow, an uneven smile or an upper face that moves too little. Nothing switches it off, but it fades as the effect wears off.

Filler placed in the wrong plane, in excess or over product that has not yet cleared can look heavy and blur the face's contours. Hyaluronic acid can be dissolved with hyaluronidase, which is kept at the clinic; calcium hydroxylapatite, polycaprolactone and poly-L-lactic acid cannot, so they are placed deep and conservatively. The serious filler risk is a blocked blood vessel: see vascular occlusion warning signs.

Both wait during pregnancy, breastfeeding and infection at the site. BTX is avoided with myasthenia gravis and similar conditions; filler is weighed carefully with a bleeding tendency, autoimmune disease, keloids or earlier non-dissolvable product.

How Atelier Decides

Examined At Rest And In Motion

Dr Sin Yong examines your face still and while you frown, smile and raise your brows. He watches which muscles create each line and compares the volume in each area with the bone beneath. That decides whether a concern belongs to BTX, to filler, to both, or to neither. Loose, redundant skin is assessed for lifting, or referred to a plastic surgeon.

Dr SY performs the examination and the injections himself, and you leave with a written plan. Suitability for filler is covered on dermal filler suitability, and what changes a quote on how we quote.

FAQ

BTX vs Dermal Fillers — Common Questions

Ask a Question
In different areas, both can often be given at one visit once the plan is clear. In the same area, BTX usually comes first, because a relaxed muscle changes how much volume the area seems to need. Dr SY sees you at the two-week check, when the toxin has reached its full effect, and only then decides whether filler is still wanted and how much.
It depends on the line. Fine vertical lines above the lip may take a very small dose of BTX, because pursing deepens them. Marionette lines and downturned corners are mostly structural, so they are approached with filler or a biostimulator to support the chin and the area in front of the jowl, often with BTX to ease the muscle that pulls the corners down.
No. Lines made by movement still respond to BTX at any age, and filler does nothing for them. What changes with age is that more faces show volume loss and descent alongside movement, so older faces are more often assessed for both injections, and for lifting or skin treatment as well. The examination decides the mix, not the birthday.
They carry different risks rather than one being safer. BTX side effects, such as a heavy brow, settle as the effect wears off. Hyaluronic acid filler can be enzyme-dissolved, yet any filler holds a small risk of blocking a blood vessel, so anatomy, depth and technique matter more than the product name. Hyaluronidase is kept at the clinic, and the warning signs are explained before you are treated.
