Dermal Filler Questions, Answered —
Short Answers First

These dermal filler questions are the ones people most often bring to a first appointment, answered briefly and linked to the page with the detail. Behind most answers sits one split, which Dr Sin Yong plans around: hyaluronic acid filler can be dissolved, and the firmer collagen-stimulating fillers cannot.

Longevity

How Long Do Fillers Last?

Longer than people often assume, and less predictably. A firm gel deep on bone, at the chin or jawline, tends to outlast a soft gel in the lips. Hyaluronic acid can stay in the tissue after its visible effect has faded, which is how layers build up when top-ups follow the calendar. Collagen-stimulating fillers run on a different clock, with collagen forming over months. The useful question is what is still there now, and that is checked by examination before anything is added.

Safety

Are Fillers Safe?

Most effects are short-lived: swelling, tenderness and bruising. Lumps, drifting product and a bluish tint are less common; nodules can appear months or years later.

The rare serious risk is filler entering or pressing on an artery, highest around the nose, frown area, forehead and tear trough, whose vessels connect with the eye's circulation. Those areas get small amounts at low pressure. A blunt cannula pushes vessels aside rather than piercing them, but neither cannula nor needle removes the risk. In Singapore, fillers are regulated by the Health Sciences Authority as medical devices; make sure the product's name is in your notes. See dermal filler complications.

Reversal

Can Fillers Be Dissolved?

Only hyaluronic acid. The enzyme hyaluronidase breaks it down; firmer, more heavily cross-linked gels break down more slowly, so dissolving often takes more than one session. It has no effect on calcium hydroxylapatite (Radiesse), polycaprolactone (Ellansé) or poly-L-lactic acid, so a lump from those is examined and managed, not dissolved. That is why people new to filler usually start with hyaluronic acid. Allergy to hyaluronidase is rare; a known allergy or a strong reaction to a bee or wasp sting is asked about first.

After Filler

How Do I Tell A Bruise From Something Serious?

A bruise is blue or purple, sits near a needle point, is tender rather than severely painful, and refills with colour when pressed. A blocked vessel shows as white, dusky or mottled skin along an artery's path, pain out of proportion, and a picture that worsens over hours.

If skin blanches or mottles, contact the clinic that injected you at once, or another doctor or an emergency department if you cannot reach it. Any change in vision, blurring, double vision or eye pain is an emergency: call 995 or go to the nearest hospital emergency department.

Bringing Dermal Filler Questions To Atelier

The first visit assesses the whole face rather than treating it. Dr SY separates lost volume from slipped tissue and skin quality, asks about every earlier filler, and arranges imaging such as ultrasound where earlier product is unclear. Photographs are kept in your medical record only. You leave with a written plan naming the areas and material, or saying that filler is not the right tool. General questions are on the main FAQ page; quotes on how we quote.

Dermal Fillers — More Questions

Ask a Question
Filler is deferred during pregnancy and breastfeeding. Its safety in that period has not been established, and because the treatment is elective there is no reason to accept an unknown risk. The same applies with an infection or cold sore near the area. A planning consultation is still fine.
Bring what you have: dates, notes, old photos and how many top-ups you had. The area is examined, and imaging such as ultrasound may show where product sits. Nothing new is layered over filler that has not been identified, because the type changes whether it can be dissolved and what is safe to add.
They can. Fish oil, vitamin E and some other supplements, like aspirin and blood-thinning medicines, make bruising more likely. Tell the clinic everything you take. Supplements may be paused beforehand if that is medically appropriate, but never stop a prescribed medicine without asking the doctor who prescribed it.
Usually it is better not to. Filler that has built up over years now helps hold the face up, and dissolving all of it at once can reveal looseness and hollowing that were hidden underneath. Correction is normally staged region by region, with time to settle in between, and only then is it decided whether anything should be added back.
Tell the clinic, even if it was many years ago. Layering new product over silicone or other non-resorbable material raises the risk of nodules, and the old material may sit where the new filler would go. If the history is uncertain, the area is scanned before anything is added.
It depends on which tissue is responsible. A shape that has flattened or a hollow points to filler. Dull, thin or dehydrated skin without a loss of contour points to a skin booster, which improves quality without adding shape. A line that only appears when you frown or smile points to BTX, which relaxes the muscle. Many faces need more than one.
