Hollow Cheeks · After Fat Removal · Pacific Plaza
Buccal fat removal regret usually arrives years after the operation, when the cheek that looked sculpted at twenty-five starts to look hollow and tired. Removed fat cannot be put back like for like. What can be done is to support and restore the tissue that remains, carefully, and to avoid treatments that take away more. This guide explains how Dr Sin Yong approaches that, and what to weigh if you are still deciding.
The buccal fat pad sits deep in the cheek, between the chewing muscles, and is a different structure from the soft fat you can pinch. Buccal fat removal takes out part of that deep pad through the mouth. Face liposuction removes fat closer to the surface. Both are surgical, and neither is performed at Atelier.
In a full, young face, taking away deep cheek fat can read as definition, and early results often please. The face, though, keeps losing volume through the thirties and forties: bone recedes, the deep fat compartments thin and the remaining buccal pad itself shrinks with age. The reserve that would have softened those changes has already gone.
The result, when regret comes, tends to be one of three things: a hollow beneath the cheekbone, a drawn or tired look at rest, or a lower face that looks older than the rest of the face. The background on why the pad matters is on buccal fat and a round face.
The Limit
Fat that has been removed is not coming back, and nothing reproduces the pad in its original position. Any honest plan starts there. The aim is not to chase what was taken, but to support what is left: lifting tissue that has descended, restoring thickness to lean areas, and adding volume only where it is genuinely missing.
It also means the first rule is to take nothing more away. A face that has already lost deep fat is the wrong place for further fat reduction, however the treatment is marketed.
The Options
Each option is discussed only after an examination of how much volume has gone and where. Most plans use more than one, in a set order, and lifting usually comes before filling.
| Option | What it does | Its limits |
|---|---|---|
| VF Lift – Vertical Facelift | Monopolar radiofrequency mapped zone by zone, with nothing injected; lifts tissue that has descended and is the protocol Dr SY uses for a lean or already hollow face | Does not replace lost volume like for like; results build over weeks |
| Hyaluronic acid filler | Adds volume on the day at the cheekbone or in the deep cheek, and can be dissolved if needed | The area under the cheekbone overfills easily; small amounts only |
| Collagen biostimulators | Prompt your own collagen over months, for broad thinning rather than a sharp edge | Cannot be dissolved; placed conservatively |
| Fat grafting | A surgical transfer of your own fat, done by a surgeon | Uncertain how much graft survives, with its own complications; not offered at Atelier |
What To Avoid
More fat reduction. Focused ultrasound lifting such as HIFU heats deep tissue, and in a lean face that heat can reduce fat that is not the target and deepen a hollow. The same caution applies to any fat-reducing energy treatment. Why that happens, and how the deep setting is kept away from thin areas, is explained in will HIFU melt face fat.
Over-correction. Filling a hollow face until the shadow disappears can turn it into a heavy one. The midface under the cheekbone takes very little before it looks unnatural, and a heavy, rounded result is its own problem, described on pillow face correction. Volume is built in small steps and judged once swelling has settled.
Still Deciding
If you have not had surgery and are considering it for a round face, the first question is where the width comes from. In many faces it is the masseter muscle, the jawbone, superficial fat or a short chin rather than the buccal pad. Each of those has a non-surgical answer that can be adjusted or stopped: jaw BTX for muscle bulk, chin or jaw definition for a short lower face, and lifting for tissue that sits low. The full reasoning is on buccal fat and a round face and V-shape face.
Where true buccal prominence is confirmed in a face unlikely to lose much more volume, a surgical opinion is reasonable, and the operating surgeon should explain the risks in full.
At Atelier
Bring what you know about the operation: when it was done, whether it was buccal fat removal, face liposuction or both, and any operative notes. Early swelling after surgery is left to settle before a hollow is judged. Dr SY then examines each layer: bone support, the position of the remaining fat, skin thickness and any filler already present, and compares the two sides. Photographs are kept in your medical record only.
The written plan sets out what can be supported, in which order, and what will not change, with no treatment as an option. How deflation in the midface is mapped is on hollow cheeks and facial volume loss; the mid-face lifting protocol is on non-surgical mid face lift.
