Non-Surgical Mid Face Lift —
Descent, Puffiness Or Hollowing

A non-surgical mid face lift at Atelier uses the VF Lift – Vertical Facelift, monopolar radiofrequency on the CLASSYS Volnewmer platform, planned for the cheeks zone by zone. Descent, puffiness and hollowing each need a different setting, so Dr Sin Yong separates them first, and dissolves filler or restores volume before any heat where that is the real problem.

The Mechanism

Three Changes, One Tired Midface

Book a Consultation

The midface ages in three ways, often at once:

  • Descent: cheek fat slides down over its ligaments, deepening the fold beside the nose and emptying the upper cheek.
  • Puffiness: the inner cheek becomes over-full, from your own fat or from filler built up over repeated top-ups.
  • Hollowing: deep fat deflates and bone recedes, so the cheek looks flat.

Volnewmer heats a volume of tissue with monopolar radiofrequency at 6.78 MHz under continuous water cooling. Each zone gets its own aim: Lift for descent, Reduce for puffiness from your own fat, and Restore for deflation. The whole protocol is on the VF Lift – Vertical Facelift page.

Method
Monopolar radiofrequency on Volnewmer, set per zone to Lift, Reduce or Restore
Sessions
Assessed at consultation; the result is judged at about three months
Downtime
Redness and warmth for hours; mild swelling or tenderness for a day or two is possible
Numbing
Usually not needed; available

Zone By Zone

What It Treats, And Who It Suits

A hollow outer cheek beside a full inner cheek is common, so one cheek often carries two settings. It suits descent with skin that still springs back, puffiness from your own fat, and early to moderate hollowing in a smaller-framed face, for people who prefer no incisions and no injected volume.

How each midface zone is assessed and set
ZoneWhat is checkedHow it is set
Outer cheekFlatness against older photographs; recoil; pinchRestore if deflated, Lift if descended
Inner cheekYour fat or old filler; ultrasound when in doubtFiller dissolved first; Reduce where the fat is yours
Fold beside the noseWhether the cheek above dropped or the area beside it filledVector runs upward; the fold itself is left unfilled
Under the eyePuffy or hollow; thin skin over the rimLight heating; hollows assessed for volume
Below the cheekboneReal deflation, or a shadow from a full inner cheekRestore only for real deflation

The Decision

Filler First, Heat Second

If puffiness comes from hyaluronic acid filler, heating it treats the wrong material. Ultrasound maps the deposits, hyaluronidase dissolves them, and the face is reassessed once swelling settles (see pillow face correction). Calcium hydroxylapatite, PCL and PLLA do not dissolve and are handled through filler correction; recent product is timed around, not heated over.

Where the shortfall is more than energy can answer, a collagen biostimulator or cheek filler is placed there.

Wait, or choose another route

  • A pacemaker or implanted electronics rule out monopolar RF; metal near the cheek changes the plan.
  • Pregnancy, or infection, inflamed acne or a cold sore in the area.
  • A very thin face: treated with restraint, as over-heating can leave a dent.
  • Hollowing from bone loss, or heavy surplus skin: volume work, or a surgical opinion by referral.

On The Day

How The Cheeks Are Mapped

  1. Photographs. Profile, three-quarter and moving views, compared with older pictures where you have them.
  2. Pinch and palpation. Fat is told apart from skin in each zone.
  3. Marking. Zones and aims are drawn.
  4. Treatment. Energy, passes and dwell time follow the tissue response and the warmth you report.

Recovery

Recovery And Aftercare

Gentle skincare, daily sunscreen, and no saunas or strenuous exercise for 24 hours. Collagen develops over 8–12 weeks, so the cheeks are compared with your photographs at about three months. The full routine is in VF Lift aftercare.

Risks

Risks And Side Effects

Redness, warmth, swelling and tenderness settle. Less often: a blister or crust from too much heat in one spot, longer-lasting altered sensation, a slight dent where fat under thin skin is over-heated, or temporary pigment change in darker skin. Report anything that does not settle the same day.

FAQ

Non-Surgical Mid Face Lift — Common Questions

Ask a Question
Not until the filler has been assessed. Hyaluronic acid causing puffiness is mapped with ultrasound and dissolved first, and the face is examined again once swelling settles. Fillers that hyaluronidase cannot reverse, such as calcium hydroxylapatite, PCL or PLLA, are timed around rather than heated over.
For some cheeks. Filler adds a product; Restore uses heat to prompt your own collagen in a deflated zone, with nothing injected. Where bone has receded or the shortfall is marked, filler or a collagen biostimulator may still be needed, sometimes combined with the radiofrequency plan.
Yes. A full inner cheek, often from filler built up over time, can make the outer cheek look hollow by comparison, and the two often sit side by side. Dissolving the excess first and then reassessing usually shows how much real hollowing is left, and only that part is treated.
It can if heat reaches the little fat a thin face has, so slim faces are treated with restraint. Restore and conservative settings are favoured, and Reduce is not used. With too little tissue to work with, Dr SY discusses volume instead.
Because the fold is usually a result, not the cause: it deepens when the cheek above descends or fullness beside it grows, and filling it tends to make the lower face look heavier. The vector runs upward from the fold towards the cheekbone, so the change comes from above.
