A V shape face is often asked for as if it were one treatment, but the taper of the lower face is set by five structures: the jaw angle, the masseter muscle, the chin, the jawline and the laxity of the tissue over it. At Atelier, Dr Sin Yong finds which of them is holding the outline back, treats that structure, and plans the steps in an order that keeps the lower face from turning heavy.
A face reads as V-shaped when it narrows from the jaw angle to a defined chin. Each structure below can interrupt that line.
Behind all five is bone. A wide mandibular angle is skeletal, and no injectable or device narrows it. Most faces combine two contributors; a wide masseter with a short chin is a common pairing in East and Southeast Asian faces.
At A Glance
There is no single V shape procedure. The plan is a ranked list of contributors, each matched to its own treatment.
Who It Suits
V shape planning suits people who find their lower face wider, rounder or softer than they want, and who accept that the result comes from a staged, assessed plan rather than one appointment.
| What limits the taper | How it is checked | What is considered | What it will not do |
|---|---|---|---|
| Enlarged masseter | Clench test at the jaw angle | Jaw BTX: less contraction, then less bulk | Alter bone, fat or loose tissue |
| Short or retruded chin | Side view against nose and lips | Chin filler on bone: firm HA, or CaHA or PCL for support | Reduce width or remove fat under the chin |
| Soft jawline | Jaw border, jowls and skin quality | Filler along the jaw angle and border | Narrow a wide face; it sharpens the edge |
| Laxity, early jowl | Pinch, recoil, tissue gently repositioned | VF Lift – Vertical Facelift, or HIFU where depth suits | Replace lost volume or shrink a full fat pad |
| Pinchable fat | Pinch at rest and in expression | Time Freeze Laser LCLR®, or double chin treatment | Touch the buccal pad deep in the cheek |
| Buccal fat pad | Fullness high in the cheek that cannot be pinched | Usually watched; removal would be surgery, by referral | Be undone; the pad deflates with age anyway |
| Wide jaw bone | Width unchanged on clenching or pinching | Proportion work on chin and jawline | Narrow bone |
The Decision
Reversing that order is a common route to an overfilled lower face, because filler adds weight to tissue that is already descending.
Women and men usually want different outlines. Women more often ask for a narrower jaw angle and a tapering chin, so masseter reduction, where the muscle is the cause, is paired with chin length or projection. Men usually want a strong jaw edge, so the masseter is often kept and the work goes into chin projection and jaw structure.
When to wait or look elsewhere. Treatment waits during pregnancy or breastfeeding, with active skin infection, and while a recent lower-face treatment elsewhere is still settling. A bite problem or a marked skeletal discrepancy is discussed before any filler and may be referred. Buccal fat removal is surgery and is referred out; be wary of any device sold as a non-surgical version of it. The face slimming guide covers the causes of a wide face.
On The Day
Recovery
Each step is judged at its own follow-up visit before the next.
Risks
Jaw BTX. Tenderness or a small bruise at the injection points; uncommonly a temporary uneven smile if toxin reaches a neighbouring muscle. Over-reducing the masseter can hollow the area below the cheekbone, which is why dosing follows the muscle rather than a template.
Filler. Swelling, bruising and small lumps. Rarely, filler blocks a blood vessel. Unusual pain, skin that turns pale, or a colour change after filler needs contact with the clinic the same day; vascular occlusion warning signs explains what to look for. Hyaluronic acid filler can be dissolved; CaHA and PCL cannot.
Energy-based lifting. Redness and tenderness are common; a small burn or blister is uncommon. In a thin face, heating can make hollows more visible.
Relapse. A treated masseter rebuilds if clenching continues, so the habit is part of the plan.
