Minoxidil —
Longer Growth, Not A DHT Blocker

Minoxidil is a hair-loss medicine used for male and female pattern thinning, as a scalp solution or foam and, off-label, as a low-dose tablet. This entry, prepared under Dr Sin Yong, covers what it does, its limits and who should take care; whether it suits you is decided at a consultation.

What It Is

A Blood Pressure Tablet That Grew Hair

Book a Consultation

Minoxidil was first developed as an oral medicine for high blood pressure, and extra hair growth was noticed as a side effect. On the scalp, an enzyme inside the follicle turns it into minoxidil sulphate, the active form.

It is thought to open potassium channels, improve blood flow around the follicle, lengthen the growth phase and let shrunken follicles make a thicker hair. It does nothing to DHT, the hormone behind pattern thinning.

The Evidence

What Minoxidil Can And Cannot Do

Placebo-controlled studies in men and women with androgenetic alopecia support it, and dermatology guidance lists it as a first-line option. Some regrow hair; many more see thinning slow and hold. Response varies, partly with how active that follicle enzyme is.

Evidence for other kinds of hair loss is thinner, so the cause is confirmed first: see male hair loss and sudden shedding.

How It Is Used

Daily, And For The Long Term

The solution or foam goes onto the scalp every day. An early shed is common (see the questions below), and results take months.

The benefit lasts only while it is used: after stopping, the hair it was supporting is lost again over the following months. How the tablet compares with the scalp form, and how it is monitored, is set out under hair loss medications.

Take Care

Who Should Take Care

  • Pregnancy and breastfeeding: not used.
  • An irritated, sunburnt or broken scalp.
  • Heart conditions or low blood pressure: check with a doctor first.

Itching, flaking or dermatitis can come from the propylene glycol in some solutions, and a foam may suit people who react. Product that runs onto the forehead can encourage fine facial hair, reported more often in women.

Singapore Status

Pharmacy Scalp Products, Prescribed Tablets

Topical minoxidil products are registered medicines, generally sold in pharmacies; the category can depend on strength. Cosmetic hair tonics may not contain it, yet the HSA has found undeclared minoxidil in some. Oral minoxidil is prescription-only.

FAQ

Minoxidil — Common Questions

Ask a Question
An early shed is common and usually means the hair cycle is changing, not that the medicine has failed. Resting hairs are pushed out so new growth can follow, typically starting within the first weeks to two months and easing over the following weeks. Stopping during the shed gives up the chance of a response. Shedding that is heavy, patchy or still going well beyond the early months, or an itchy, inflamed scalp, should be examined.
It is less studied at the front than at the crown, and medical treatment tends to preserve a hairline rather than rebuild one. If the front is already lost, a surgical opinion may be discussed. Hair transplantation is a surgical procedure. At Atelier, Dr SY assesses whether and when a transplant makes sense and plans the medical treatment around it.
Only through a doctor. For hair loss, oral minoxidil is prescribed off-label, and because it acts on the whole body the doctor first checks blood pressure, heart history and current medicines. Fluid retention, a faster heartbeat, light-headedness and extra body hair are discussed before starting. Whether it suits you, or the scalp form is the better choice, is decided at a consultation.
