Forehead Lines —
Read The Brow First

Horizontal forehead lines come from the frontalis, the muscle that lifts your brows. This page is about the brow beneath them: how its height and the upper lids decide how much the forehead can be relaxed, and what helps lines that have set into the skin.

The Mechanism

Why The Forehead Works So Hard

Book a Consultation

Stretching from hairline to brow, the frontalis is the one muscle that lifts the brows. Each lift folds the skin into horizontal ridges, which young skin smooths out once the muscle lets go.

How hard the frontalis works depends on what it is pulling against.

Driver One
Expression
Frequent brow-raisers, animated talkers and people who lift their brows to concentrate tend to show lines earliest.
Driver Two
Strong Depressors
The corrugators, procerus and outer orbicularis oculi pull the brows down. Strong depressors make the frontalis work harder, and the lines deepen.
Driver Three
Heavy Upper Lids
Some foreheads keep lifting to hold heavy or hooded lids off the lashes. The lines are a by-product, and full relaxation lets brows and lids drop.
Driver Four
Thinning Skin
As age and sun thin the collagen, each fold lingers longer, faintly at first and later even at rest.

Types

What The Brow Tells You

Before the lines, Dr Sin Yong looks at the brow. At rest it normally sits at or just above the bony rim of the eye socket, its tail a little higher than its inner end. A low brow, or one that jumps up when you look ahead with a slack forehead, suggests compensation.

A simple test helps: close your eyes gently, let the forehead go slack, then open them without lifting your brows. The lid now resting on your lashes is what the forehead has been holding up. The upper lids are also checked for excess skin and for a margin sitting low over the pupil.

Specifications

Region
Forehead: the frontalis, its balance with the brow depressors, and the upper lids it may be supporting
Assessment
Brow height at rest, full raise, frown, eyes closed then reopened; upper-lid skin and lid margin; side-to-side difference
Modalities
Pattern-mapped BTX, depressor balancing, skin-directed treatment for etched lines, energy-based brow lifting, referral
Review
BTX checked at 2 weeks; any repeat timed after brow position and movement are re-examined
Type I
Comfortable Brow
Lines show on raising and fade at rest, and the brows sit easily unaided. Conservative, mapped BTX suits this pattern.
Type II
Low Or Lid-Bearing Brow
The brow sits at or below the bony rim, or rises as the eyes open, and the lids feel heavy when the forehead relaxes. The lower forehead is spared and the lids are dealt with first.
Type III
Uneven Raise
One brow habitually sits higher, with deeper lines on that side. Each side is dosed separately and the outer frontalis balanced; matching doses do not help.

The Decision

Brow First, Lines Second

The frontalis is the brow's only lifter, so a fully relaxed forehead lets the brows settle. In a low brow, or a forehead holding up heavy lids, that reads as heaviness and can push the hood onto the lashes. The brow, not the lines, sets the limit on the dose.

When the forehead is compensating, the plan is cautious and often redirected. The lower frontalis is left working and only the upper forehead softened. The muscles that pull the brows down may be treated so they stop dragging them lower. Where heavy lids are why the forehead works, energy-based lifting aimed at brow and lid, or an eyelid referral, comes before stilling the forehead.

For a comfortable brow, small mapped doses of BTX, a prescription medicine that reduces the nerve signal to the muscle, go across the upper and middle forehead, adjusted for asymmetry. The aim is softer movement, not a forehead that cannot move. The frown is usually treated at the same visit; see forehead and frown lines.

When to wait. Treatment is put off in pregnancy or breastfeeding, with infection in the area, with a neuromuscular disorder such as myasthenia gravis, after an allergic reaction to any botulinum toxin product, and until medicines affecting nerve-to-muscle transmission are checked.

When to refer. A heavy lid caused by the lid itself is a separate problem and is referred to an oculoplastic or plastic surgeon.

At Atelier

When The Crease Is In The Skin

A line still visible with the forehead relaxed means the dermis has changed. BTX can be the first stage of the plan, but on its own it does not soften an etched crease. The options at Atelier are:

  • BTX for movement, as set out on BTX for wrinkles and prevention.
  • RF microneedling or fractional laser to remodel the creased dermis. See RF microneedling.
  • Skin boosters and polynucleotides to thicken thin forehead skin. See skin boosters.
  • Forehead filler, only for deep folds at rest or a flat, hollow forehead, never for movement lines. The forehead is a high-caution area because of the vessels beneath it, and filler is planned separately from BTX after examination. See forehead filler.
  • Energy-based brow lifting with focused ultrasound or radiofrequency, aimed at brow position where that is the cause. See non-surgical eyelid lift and HIFU.

Safety

After Forehead BTX

Small bruises, tenderness and occasionally a mild headache are common and pass. Three risks belong to the forehead. Too much relaxation for the lids to cope with can leave the brow low or heavy. Medicine reaching the lid-lifting muscle can make an eyelid droop. And if the outer forehead keeps moving while the centre is quiet, one outer brow can arch upward.

Each lasts while the medicine acts and cannot be switched off sooner, though a raised brow can sometimes be evened out (see the questions below). Brow assessment, mapped placement and conservative doses reduce these risks without removing them.

Call the clinic if a drooping lid or brow troubles you, if the two sides look clearly different, or if you see double. Difficulty swallowing, speaking or breathing after BTX needs urgent care at a hospital emergency department: go straight there rather than waiting to reach the clinic.

FAQ

Forehead Lines — Common Questions

Ask a Question
Because the forehead may be doing a second job. In some people the frontalis keeps lifting to hold heavy or hooded lids off the lashes. Relax it fully and the brows settle, so the lids feel heavier. Checking the lids first shows Dr SY how much of the forehead can be softened, and whether the lids need their own plan.
Possibly, but with a different plan. Only the upper forehead is softened, with small doses, and the lower part is left active. If brow position is the real issue, energy-based brow lifting or referral to an oculoplastic or plastic surgeon may help more than further forehead BTX, which can let the brow sit lower still.
Not always, but treating the forehead alone is often unwise. The frown muscles pull the brows down, so relaxing only the frontalis can leave them unopposed and let the brows drop. Treating the frown at the same visit keeps both sides of the balance even, so forehead and frown are usually planned together.
An arched outer brow appears when the outer forehead keeps moving while the centre is quiet. It lasts only while the medicine acts, and sometimes another small dose, decided at your follow-up visit, evens it out. Next time each side is dosed on its own, because matching doses do not suit a forehead that moves unevenly.
See a doctor if your lines come with heavy or drooping lids, if one brow starts to sit lower than the other, if lines at rest keep deepening despite sun protection, or if headache, eye strain or squinting starts alongside them. A new brow or lid droop on one side needs a medical assessment, not a cosmetic appointment.
No. Every contraction of the frontalis folds the forehead skin, so exercising it adds creasing rather than removing it. Sun protection and skincare support the dermis; movement is addressed with BTX where your brow allows, and etched lines with skin treatment.
