Shots And Lines · Depth First · Pacific Plaza
HIFU treatment is often described by its shot count, as if 300 or 800 shots were a fixed dose. A count leaves out the depth, the energy at each point, the device and the areas covered, so one number can describe very different treatments. This article explains what shots and lines are, why counts vary, and what a useful plan lists instead.
A shot is one focal point: a tiny spot of tissue brought to roughly 60–70 °C at a set depth, while the skin above it is spared. As the handpiece moves, shots placed one after another form a line, and the lines together make a map across the face and neck.
Not every platform works in discrete shots. Some lay energy along a line as rapid micro-pulses while the handpiece glides, and image-guided systems such as Ultherapy plan each line against a live picture of the layers. So a fair first question about any number is what it counts: points, lines, pulses or energy, and at which depth. Machines and cartridges are compared in the HIFU Singapore guide.
Dose
The heat delivered is the number of points multiplied by the energy at each. A high count at low energy and a low count at high energy are different treatments that can carry similar numbers.
HIFU is meant for mild to moderate looseness in particular layers. Once that need is met, extra energy adds heat without adding lift. In a lean face, surplus lines can thin the fat that holds the cheek's shape, as explained in will HIFU melt face fat? A heavy lower face with skin that folds is beyond what any count can reach.
Energy is also adjusted during treatment to what you feel. The deepest setting is usually felt most, often as a deep warmth or brief ache over the jawline. A count fixed in advance sits awkwardly with that adjustment.
What The Count Leaves Out
One hundred shots placed in the dermis and one hundred placed in the SMAS reach different tissue, feel different and carry different risks. The 4.5 mm depth needs enough tissue above bone, and nothing is placed over the thyroid, the bony rim around the eye or the nerve path along the jaw border. Brow and forehead lines use shallow settings.
Lines are spread unevenly on purpose. One zone may take several depths, another none. The count is an output of the plan, built from the area, the tissue thickness, the suitable depths and the degree of looseness. Where volume loss or a full fat pad is the real change, the answer is a different approach, not more lines.
| Factor | Why it changes the treatment |
|---|---|
| Depth | A 1.5 mm point and a 4.5 mm point heat different layers; Atelier uses 1.5, 3.0 and 4.5 mm |
| Transducer frequency | Changes how deep the focus sits and how tight it is; lower frequencies reach deeper |
| Energy per point or line | The same count at a different energy is a different dose |
| Delivery pattern | Shot-based, micro-pulsed and image-guided platforms do not count in the same unit |
| Area and thickness | A narrow face with thin tissue over bone needs a different spread from a broader one |
| Visit or course | A figure may cover one visit or several, one area or the whole face and neck |
Between Clinics
Figures seen online rarely name the device, depths, energy or areas. The same number on two devices, two cartridge ranges or two energy settings is not the same treatment, and on one device it means something different for the jaw alone than for the full face and neck. A lower number may simply reflect a different device, fewer zones, shallower depths or a preset plan.
Research does not support counting either. A small study of twenty people treated across seven facial areas found assessments similar whatever the shot count, though it covered a single session and does not settle the question. A systematic review found that settings, protocols and energies varied widely between studies, and that results were smaller where laxity was greater.
At Atelier
Ask about the plan rather than the number:
At Atelier, Dr Sin Yong examines first and gives a written plan listing the areas, depths, device and follow-up; the quote follows from that plan, as how we quote explains. The change builds as collagen remodels over 8–12 weeks and is judged at about three months; early firmness is not the result. How often treatment is repeated is covered in how many HIFU sessions, and the treatment itself on the HIFU treatment page.
