Eye Bags vs Dark Circles vs Tear Trough —
How They Differ

Eye bags vs dark circles vs tear trough: three problems that sit within a centimetre of each other and are often called by each other's names. A bag is a bulge, a dark circle is a colour, and a tear trough is a groove. This guide puts them side by side so you can see why the treatment for one can make another look worse.

How Each Forms

A Bulge, A Colour And A Groove

The lower lid holds three pads of orbital fat behind a thin sheet called the septum. Below them, a ligament ties the skin to the bony rim of the eye socket. The skin over all of it is very thin. Each of the three problems starts in a different one of these layers.

Eye Bag
Fat Moving Forward
With age the septum slackens and orbital fat pushes forward into the lower lid. A fat bag is still present at 8 a.m., even after a full night's sleep, and grows when you look up. Puffiness that is worst on waking and fades by afternoon is fluid, a different problem.
Dark Circle
Colour In Or Under The Skin
Brown circles are melanin in the lid skin, driven by genes, rubbing, eczema and sun. Blue-violet circles are vessels seen through thin skin. Either can exist with no bag and no groove at all.
Tear Trough
A Groove On The Rim
Where the ligament holds skin to bone, a groove forms at the junction of lid and cheek. As the midface loses volume it deepens, and overhead light drops a shadow into it. The skin is not darker; the contour is.

The first rows are checks you can try in a mirror. They are screening observations, not a diagnosis. The later rows show what each finding usually means for treatment.

Which Suits Which

Why The Wrong Treatment Backfires

The tear trough is a structure; a dark circle is an appearance. They overlap only when the darkness is the groove's own shadow. If the darkness is pigment, vessels or a bag sitting above the groove, the trough itself may be unremarkable and filler has nothing to correct.

  • Filler beside a fat bag adds weight in front of the bulge and can make it more visible. So-called eye bag filler is not a treatment for the bag; at most, a small amount in the groove below a modest bag can soften the step.
  • Brightening creams and pigment lasers do nothing for a shadow, and bleaching creams have nothing to act on in a vascular circle.
  • Filler over pigmented skin can add puffiness without changing the colour.
  • Creams, caffeine rollers and tape do not move fat. Tape lets go as soon as it comes off.
  • More sleep helps fluid. It does not shrink a fat bag or fill a groove.

Puffiness that sits on the upper cheek below the rim is a separate condition, malar bags and festoons, and is assessed on its own terms. For a closer look at the colour types, see which type of dark eye circles you have; for the bulge, what causes eye bags.

How Atelier Decides

One Eye, Often All Three

After forty, a common picture is a mild bag, a deepening trough and some brown or blue colour in the same eye. Dr Sin Yong examines you upright, looking up, with the head tilted back, with gentle pressure on the closed upper lid, and with the lower-lid skin stretched. Treatment then follows the component that contributes most:

  1. Stop rubbing and protect from sun when pigment leads.
  2. Skin-quality work when thin skin over vessels leads.
  3. Volume when a true trough leads, using tear trough filler.
  4. Referral when a substantial fat bulge leads. Lower eyelid surgery is not performed at Atelier, and the assessment first confirms that fat, rather than a hollow, fluid or loose skin, is the cause.

Treatment options by colour type are on dark eye circle treatment, and non-surgical options for under-eye fullness on eye bag treatment.

See a doctor first if swelling is new, on one side or worsening quickly; if puffiness comes with redness, pain, itching or a change in vision; or if it persists with swelling elsewhere or unexplained tiredness.

FAQ

Eye Bags, Dark Circles And Tear Troughs — Common Questions

Ask a Question
Look at shape versus colour. An eye bag changes the shape of the lower lid: there is a bulge, and it often grows when you look up. A dark circle changes the colour or shading of the skin with no bulge needed. Many people have both, and a bag can also throw its own shadow onto the skin below, which is why an examination under different light is more reliable than a mirror.
Because it is probably structural. Sleep, less salt and less alcohol reduce fluid, which is why puffiness that is worst on waking and fades through the day is usually fluid. A bulge that is present at breakfast every day, after good sleep, suggests orbital fat that has moved forward past a weakened septum. Fat does not respond to rest, creams or rollers, so it is assessed rather than waited out.
Sometimes, but it treats the groove, not the bag. Where a modest bulge sits above a hollow, a small amount of hyaluronic acid placed deep in the groove can soften the step between lid and cheek. The bag itself does not shrink. Over a larger fat bulge, filler adds weight and can make the bulge more obvious, so a surgical opinion is the more useful step there.
No. The tear trough is a groove in the anatomy, while a dark circle is something you see. The two overlap only when the darkness is the shadow cast by the groove. If the colour is pigment or visible vessels, filling the trough changes the contour but leaves the colour as it was, which is a common reason under-eye treatment disappoints.
