Bacne · Staged Care · Pacific Plaza
This page covers acne treatment in Singapore for the back, chest and shoulders: how a course is planned and run at Atelier. Dr Sin Yong confirms what the bumps actually are, clears active acne with treatment matched to that diagnosis, and only then turns to the marks and scars it has left.
Why the trunk behaves differently from the face, with larger follicles, thicker skin and a greater tendency to raised scars, is explained on back and body acne. This page is about what is done about it.
A back acne plan has three stages, and skipping the first is the usual reason months of products achieve little.
Specifications
Medical treatment does the main work on the trunk. Devices come later, for what acne leaves behind.
Who It Suits
Treatment suits breakouts that keep returning or have started to scar, itchy bumps once their cause is confirmed, and marks or scars once the acne has cleared. The first visit sorts what is on your skin, because each finding leads somewhere different.
| What is found | Likely cause | First step |
|---|---|---|
| Blackheads with spots and lumps of different sizes | Truncal acne | A medicated wash and topical treatment; oral treatment for widespread or deep acne |
| Small even bumps that itch and flare with sweat, no blackheads | Malassezia (yeast) folliculitis | Antifungal treatment, not acne antibiotics |
| Pustules centred on hairs after shaving or friction | Bacterial folliculitis | Treated as an infection; a swab if it keeps returning |
| Rough, evenly spaced bumps on the arms or thighs | Keratosis pilaris | Not acne; managed separately |
| Deep, recurring lumps in the armpits, groin or under the breasts | Possible hidradenitis suppurativa | Referral to a dermatologist |
| Firm raised scars on the chest or shoulders | Hypertrophic scar or keloid | The keloid pathway, not resurfacing |
The Decision
Reach is the main problem on the back. A cream twice a day across both shoulder blades is hard to apply and hard to keep up, so a benzoyl peroxide wash left on for the advised contact time before rinsing often replaces it, with a topical retinoid where it can be reached. Widespread, deep or scarring acne on the trunk moves to oral treatment sooner than the same acne on the face: a limited course of an oral antibiotic such as doxycycline alongside benzoyl peroxide, hormonal treatment for some women, or isotretinoin for severe, nodular or scarring acne, with monitoring and pregnancy prevention. Prescription medicines are discussed only after examination, and whether one suits you is decided at a consultation. The medical options for acne in general are on acne treatment.
When to wait. Oral treatment is reconsidered during pregnancy or while trying to conceive (see acne in pregnancy), with a known drug allergy, and with liver or other conditions that limit oral medicines. Laser work waits after recent isotretinoin; timing is set individually, as in isotretinoin and laser timing.
When to refer. Acne that stays severe or keeps scarring despite standard treatment, a diagnosis that remains uncertain, and suspected hidradenitis suppurativa may need referral to a dermatologist.
On The Day
Between Visits
Shower soon after exercise and change out of damp clothes. Rinse hair conditioner off before washing your back, leave a medicated wash on for its contact time, pat dry, and choose breathable fabrics. Heavy body oils are better avoided. Benzoyl peroxide bleaches fabric, so white towels make sense.
Once the skin is quiet, the marks are addressed. Brown marks on the trunk fade more slowly than on the face; a carbon laser peel can help an oily, congested back between flares but is not a scar treatment. Depressed scars follow the facial sequence with more caution, using subcision, TCA CROSS and fractional resurfacing; see acne scar treatment and acne scar resurfacing. Raised scars go to keloid treatment.
Safety
Expected: dryness and irritation from washes and retinoids, which are started gently; bleached towels and clothes; stomach upset or sun sensitivity with some oral antibiotics; dry lips and skin with isotretinoin, which also needs blood tests and must not be taken in pregnancy.
Contact the clinic the same day for redness that spreads quickly, increasing pain or pus, fever, a rash or swelling after starting a new medicine, low mood or mood changes, or a severe headache while on oral treatment. Swelling of the lips or face with difficulty breathing is an emergency: call 995.
