A Clinical Guide to What Filler Can and Cannot Do

Dermal filler is not a wrinkle eraser — it is a structural tool. This guide explains what filler restores, which products suit which anatomical plane, and why restraint produces better results than volume.

1. Filler restores structure, it does not erase lines

Volume loss is one of three pillars of facial ageing, alongside structural descent and skin quality decline. Filler addresses the first of these and nothing else. A patient whose primary problem is descent will not be helped by adding volume — they will simply become a heavier version of the same shape. This is why assessment matters more than product selection.

2. Product choice follows the anatomical plane

High-viscosity fillers belong deep, on or near periosteum, where they provide structural projection. Low-viscosity products belong superficially, where thin skin will not tolerate a firm bolus. Placing a firm filler shallowly is the single most common cause of visible lumping and the Tyndall effect.

3. Collagen stimulators behave differently from hyaluronic acid

Radiesse and Ellanse do not simply occupy space — they provoke a fibroblast response that lays down the patient's own collagen over months. That makes them well suited to structural restoration but poorly suited to areas requiring reversibility. Hyaluronic acid remains the choice wherever hyaluronidase reversal may be needed.

4. The volume that looks best is less than the anatomy will accept

A face will physically accommodate far more filler than it should receive. Overfilling is not usually a single dramatic event — it accumulates across sessions as each new syringe is judged against an already-augmented baseline rather than the original face.
